Objective1.To describe the fall fear,quality of life and their correlation in elderly stroke patients.2.To analyze the influencing factors of fall fear in elderly stroke patients.3.To explore the effect of coach back health education on fall fear,quality of life and balance function in elderly stroke patients.Methods1.A total of 230 elderly patients with stroke who were hospitalized in the neurorehabilitation department of a third class hospital in Tianjin from June 2019 to December 2019 were selected as the research subjects.The general data questionnaire,the revised version of the Modified Falls Efficacy Scale and 12-Item Short Form Health Survey were used conduct a cross-sectional survey.2.Sixty-four elderly patients with stroke who were hospitalized with FOF in the neurorehabilitation department of a third class hospital in Tianjin from June 2019 to December 2019 were selected as the research subjects.The east area of the neurorehabilitation department is the intervention group,and the west area of the neurorehabilitation department is the control group.The control group is provided with traditional health education.The intervention group is implemented the Teach-back method based on the control group.The interventional effect of the Teach-back method was evaluated before the intervention,14 days after the intervention,and 3 months after the intervention by using the Modified Falls Efficacy Scale,12-Item Short Form Health Survey,and Time Up and Go Test.3.Using Excel software to enter the verification data for two persons,and spss24.0software to carry out statistical analysis,P < 0.05 indicates that the difference is statistically significant.The main statistical methods used are: descriptive analysis,two independent samples t-test,one-way ANOVA,multiple stepwise regression analysis,pearson correlation analysis,chi square test,repeated measurement ANOVA and so on.Results1.A total of 235 questionnaires were distributed in this study,and 235 were recovered with a 100% recovery rate.After removing 5 invalid questionnaires,230 valid questionnaireswere obtained,with an effective recovery rate of 97.87%.The results of the study showed that the total fall efficacy score of elderly stroke patients was(86.70±18.32)points,and the total quality of life score was(56.05±17.78)points.The univariate analysis results showed that:different ages,marital status,living status,education level,average monthly income of the family,whether the stroke first occurred,the course of the stroke,whether there were other diseases diagnosed,the type of medication taken during hospitalization,and whether there was a history of falls There was a statistically significant difference in the fall efficacy scores of elderly stroke patients with and without assist devices and exercise frequency(P <0.05).Multivariate stepwise regression results showed that the four independent variables:medication type,number of physical exercises,stroke duration,and living conditions during the hospitalization entered the regression equation,which together explained the 32.3%variation in the fall efficacy of elderly stroke patients(F = 21.372,P = 0.000).There was a positive correlation between fall efficacy and total quality of life scores(r = 0.871,P <0.001).2.There were no significant differences in general information,fall efficacy scores,quality of life scores,and timed time up and go test between the two groups of patients before the intervention(P> 0.05),which were comparable.The results of t-test of two independent samples showed that there were significant differences in fall efficacy between the intervention group and the control group after 14 days and 3 months of intervention(P < 0.05).The results of repeated measurement analysis of variance showed that the differences in time main effect,intervention main effect and interaction effect were statistically significant(P < 0.05).The results of t-test of two independent samples showed that there was no significant difference in the total score of quality of life between the intervention group and the control group after 14 days of intervention(P > 0.05),but only significant difference in the overall health(P < 0.05);three months after intervention,the total score of quality of life,physiological function,physiological function,overall health,vitality,social function,emotional function and mental health of the two groups were statistically different All of them were significant(P < 0.05).The results of repeated measurement analysis of variance showed that there was significant difference between the main effect of time and the main effect of intervention(P < 0.05).The results of t-test of two independent samples showed that there was no significant difference in balance function between the intervention group and the control group after 14 days of intervention(P> 0.05);there was significant difference in balance function between the two groups after 3 months of intervention(P < 0.05).The results of repeated measurement analysis of variance showed that there was significant difference between time main effect and interaction effect(P < 0.05).Conclusions1.Some elderly patients with stroke have a fear of falling.The higher the fall efficiency,the higher the quality of life.The types of medications taken during hospitalization,the number of physical exercises,the duration of stroke,and the living conditions are the main factors affecting the effectiveness of falls.2.Teach-back method can improve the fall efficacy,quality of life and balance function of elderly stroke patients.
