Application Of Teach-back Method In Health Education Of Permanent Stoma Patients

Objective: To explore the clinical effect of teach-back method in the health education of permanent stoma patients,and to provide reference for the establishment of health education strategies for permanent stoma patientsMethods: 66 permanent stoma patients who met the inclusion criteria and were admitted to the gastrointestinal surgery department of The First Affiliated Hospital of Guangxi University of Chinese Medicine,from January2019 to December 2019 were selected.The control group was guided by the regular health education method according to the developed health education program,and the observation group was guided by the teach-back method according to the developed health education program.The intervention time was from admission to 3 months after discharge.The level of self-care,self-efficacy and hope-quality of life-ostomy of the patients were rated 1 day after surgery.At the time of discharge,the patients' readiness for hospital discharge,quality of discharge teaching,self-care level,self-efficacy and hope-quality of life-ostomy were rated.Three months after discharge,the self-care level,self-efficacy and hope-quality of life-ostomy of the patients were scored again,and the unplanned readmission rate of the patients was calculated through the follow-up results.Epidata3.1 was used to input data and spss22.0 was used to analyze the data.According to the nature of the data,frequency,mean ±standard deviation were used for statistical description.T-test,chi-square test,Fisher exact test,rank sum test and variance analysis were used for statistical inference.Results:(1)65 patients were included in this study,including 32 in the observation group and 33 in the control group.There was no significant difference in general demographic data and disease-related data between the two groups(P > 0.05),comparability;(2)Comparison of readiness for hospital discharge between the two groups: at discharge,the readiness for hospital discharge of the observation group was higher than that of the control group,with statistically significant differences(t=-9.793,P < 0.05);(3)Comparison of quality of discharge teaching between the two groups: at discharge,the quality of discharge teaching of the observation group was higher than that of the control group,with statistically significant differences(t=-9.409,P < 0.05);(4)Comparison of self-care ability between the two groups: 1 day after surgery,there was no statistically significant difference in self-care ability between the two groups(t=0.234,P > 0.05).At discharge time and three months after discharge,the self-care ability score of the observation group was higher than that of the control group,and the difference was statistically significant(t=-4.224,P < 0.05;t =-7.889,P <0.05);(5)Comparison of self-efficacy between the two groups: 1 day after surgery,there was no statistically significant difference in self-efficacy between the two groups(t=0.432,P > 0.05).At discharge time and 3 months after discharge,the self-efficacy score of the observation group was higher than that of the control group,and the difference was statistically significant(t=-7.047,P < 0.05;t =-9.564,P < 0.05);(6)Comparison of hope-quality of life-ostomy between the two groups: 1 day after the surgery,there was no statistically significant difference in hope-quality of life-ostomy between the two groups(t=-0.577,P > 0.05).At discharge time and three months after discharge,the hope-quality of life-ostomy score of the observation group was higher than that of the control group,and thedifference was statistically significant(t=-4.823,P < 0.05;t =-6.008,P <0.05);(7)Comparison of readmission rate of patients due to stoma nursing problems between the two groups: 3 months after discharge,the readmission rate of the observation group was lower than that of the control group,and the difference was statistically significant(P < 0.05).Conclusions:(1)Health education for permanent stoma patients by teach-back method can significantly improve the self-care ability,self-efficacy and hope-quality of life-ostomy of patients at the time of discharge and three months after discharge compared with the conventional health education method.(2)Teach-back method was used to conduct health education for permanent stoma patients,and the patients' readiness for hospital discharge and quality of discharge teaching were higher than the conventional health education.(3)Health education for permanent stoma patients by teach-back method can significantly reduce the rate of readmission of permanent stoma patients.
