Clinical Observation On Treating The Urinary Retention During Recovery Period Of The Cerebral Ischemic Stroke By The Fire Needle Therapy Combined With Acupuncture At The Scalp Foot Motor Sensory Area

Objective:Study the effectiveness and safety of the fire needle combined with acupuncture at the Foot motor-sensory area on treating the urinary retention during recovery period of the cerebral ischemic stroke through a randomized controlled clinical study,to evaluate the differences and advantages of fire needle therapy and millipuncture,which can provide some reference for formulating and optimizing the clinical treatment plan of the urinary retention during recovery period of the cerebral ischemic stroke.Methods:Using a randomized control design,60 patients with urinary retention during the recovery period of ischemic stroke who met the inclusion criteria were divided into 2 groups,and 30 cases in each of the treatment group and the control group.There were no significant differences in the general data:age,sex,course of disease.And there were no significant differences in residual urine volume of bladder,bladder function score,and urinary retention before treatment.First of all,the treatment of the two groups of patients refers to the relevant guidelines to improve cerebral blood circulation,nutritional nerves,and carry out secondary prevention of cerebrovascular disease actively.Monitor vital signs,develop an individualized treatment plan based on the patient's own basic situation,and use drugs that do not affect urination to control blood pressure,blood sugar,and blood lipids.On this basis,according to the residual urine volume of the patient's B ultrasound,it is judged whether to indwell the urinary catheter.When the residual urine volume exceeds 200 ml clinically,it is recommended to keep the urinary catheter,generally not more than 7 days,and regularly update the urinary catheter according to the patient's own situation.Besides,the patients in both groups retained the urinary catheter and performed daily bladder function training.All the traditional Chinese medicine treatments use conventional acupuncture to treat limb symptoms of stroke,and select the foot motor sensory area and Shenshu,Pangguangshu,Sanjiaoshu,Zhongji,Guanyuan,Shimen and Sanyinjiao points to treat urinary retention symptoms.In the treatment group,acupuncture at Shenshu,Pangguangshu,Sanjiaoshu,Zhongji,Guanyuan,Shimen and Sanyinjiao acupuncture points was performed 3 times/week/course of treatment.The control group used millipuncture to acupuncture acupoint of the above body,once/day,6times/week/course of treatment.Both groups were given 2 courses of treatment.Observe,record and compare the residual urine volume of bladder,bladder functionscore,and urinary retention before and after treatment of the two groups of patients,refer to Standards for Diagnosis and Treatment of Traditional Chinese Medicine Diseases written by the State Administration of Traditional Chinese Medicine,supplemented with B-ultrasound examination,and evaluate the fire needle therapy combined with acupuncture at the Foot motor-sensory area in the treatment of urinary retention during the recovery period of the cerebral ischemic stroke,and compared with the conventional control method of millipuncture.During the treatment,if the patient has a strong sense of urination,obvious urination consciously,or urinary tract urinary tract leakage for 2 to 3 consecutive times,try to encourage the patient to urinate autonomously.If the patient urinates smoothly within24 h,there is no discomfort,and the residual urine of B-mode ultrasound is less than 50 ml,it can be considered that the voluntary urination is successful,and the treatment is ended;If the patient can urinate on his own,but the urination is not smooth,only drip out,or(and)the residual urine of B-mode ultrasound>50ml,then reindwell the urinary catheter and continue to be treated with acupuncture until the end of the treatment course.Results:(1)According to the evaluation standard of curative effect of traditional Chinese medicine,after treatment,the total effective rate of the treatment group was 93.33%,which was higher than that of the control group80.00%(P<0.05);According to the evaluation standard of modern medical urinary retention efficacy,the two groups of efficacy in comparison,the total effective rate in the treatment group was 90.00% which was higher than that in the control group76.67%,the difference was statistically significant(P<0.05).(2)The residual urine volume of the two groups of patients after treatment was lower than that before the treatment,whose difference was statistically significant(P<0.05);After treatment,the residual urine improvement of the treatment group was better than that of the control group,with statistical significance(P<0.05);(3)The bladder function scores of the two groups were reduced after treatment,and the difference was statistically significant(P<0.05);After treatment,the reduction in bladder function scores of the treatment group was better than that of the control group,and the difference was statistically significant(P<0.05).(4)The degree of urinary retention after treatment in both groups was better than before,whose difference was statistically significant(P<0.05);The comparison between the groups after treatment,the symptom improvement of the treatment group was better than the control group,there was a significant improvement,and the difference was statisticallysignificant(P<0.05).Conclusions:1.For urinary retention after ischemic stroke,fire needle acupuncture and millipuncture have clinical effects.2.The overall effect of fire needle therapy with acupuncture at the Foot motor-sensory area in the treatment of urinary retention during recovery period of the cerebral ischemic stroke is better than that in the contral group,which can significantly reduce residual urine volume,reduce bladder function score,and relieve urinary retention.3.Fire acupuncture therapy is safe,reliable and effective.The treatment time is short,the number of acupuncture times is less,and the treatment interval is longer than that of the general millipuncture treatment.There is much room for clinical development and it is worth promoting.
