Purpose:To observe and compare the clinical effect and safety of "knee seven knives" acupotomy, postoperative prescription of knee joint and "knee seven knives" combined with postoperative prescription of knee joint in the treatment of knee osteoarthritis with liver-kidney deficiency,so as to provide a new therapeutic idea for the clinical treatment of this disease. Methods:In the outpatient department of rheumatoid osteopathology of gansu hospital of traditional Chinese medicine,90 cases of patients with knee osteoarthritis were screened and randomly divided into acupotomology group, adopting "knee seven knives" cure.In the traditional Chinese medicine group, eating postoperative prescription of knee join,1 dose/day was taken twice in the morning and evening. And the combination group, the combination treatment.Thirty patients in each group were treated for three weeks in all three groups. Before, after and 4 weeks of follow-up after treatment, the changes of TCM syndrome score,WOMAC osteoarthritis index score, visual analogue scale(VAS)of pain and other related indicators and main symptoms and signs of the three groups were recorded and analyzed, and the safety was evaluated.SPSS 24.0 software was used for statistical analysis of the collected data.P<0.05 was used to determine whether the difference was statistically significant. In the measurement data, the centralized trend and the discrete trend are represented by the mean standard deviation((?)ħS),and the non-normal distribution by the median interquartile distance(M ħIQR).Count data are expressed as ratios or frequencies.One-way ANOVA or Kruskal Wallis non-parametric test were used for comparison among the three groups of measurement data.Independent sample t-test or Z-test were used for comparison between the two groups.Paired t-test or signed rank sum test were used for comparison of data before and after treatment in each group.Chi-square test was used for non-grade counting data, and Kruscal-Wallis H test for grade data with multiple independent samples.Results:(1) Score of TCM symptoms:after treatment, the Acupotomy group and the combined group improved significantly in pain, swelling, joint function limitation and other aspectscompared with before treatment(P<0.01),and the efficacy was durable;the combined group and the traditional Chinese medicine group improved in swelling, lumbar and knee weakness, hot and night sweats compared with before treatment(P<0.05),and the efficacy was durable;the combined group was superior to the Acupotomy group in lumbar and knee weakness and hot and night sweats(P<0.05),better than the traditional Chinese medicine group in terms of pain and joint function limitation(P<0.01).(2)VAS scores:after treatment, the scores of the three groups decreased compared with those before treatment(P<0.05);after 4 weeks of follow-up,the scores of the needle-knife group and the combined group decreased compared with those before treatment(P<0.05);there was no significant difference between the needle-knife group and the combined group(P>0.05),and the relief of knee pain in the combined group and the needle-knife group was better than that in the traditional Chinese medicine group(P<0.01).(3)In terms of WOMAC osteoarthritis index:after treatment, each score of the three groups was lower than that before treatment(P<0.01);after 4 weeks of follow-up,each score of the needle-knife group and the combined group was lower than that before treatment(P<0.01);there was no significant difference between the needle-knife group and the combined group(P>0.05),and the improvement of the combined group and the needle-knife group was better than that of the traditional Chinese medicine group(P<0.01).(4)In terms of the curative effect of TCM symptoms:the total effective rate was 80.00%in the acupotomy group,66.67% in the traditional Chinese medicine group, and 86.67% in the combined group, with a statistically significant difference(P<0.05).(5)No serious adverse reactions were found in the three groups. Conclusion:"Knee seven knives" therapy, postoperative prescription of knee joint, and "knee seven knives" combined with postoperative prescription of knee joint all have certain effects on knee osteoarthritis with deficiency of liver and kidney."Knee seven knives" combin ed with postoperative prescription of knee joint, will combine the advantages of "seven knives of the knee" with the advantages of the knee joint after surgery, take advantage of the strengths and weaknesses, and can effectively alleviate the clinical symptoms suc h as knee pain, swelling, limited joint movement function and hot flashes and night sweats, with a long-lasting effect, while no serious adverse reactions occur, which can be use d as a new treatment idea for this disease.
