Objectives:To compare the changes of clinical scores and therapeutic effects of acupotomy in patients with knee osteoarthritis(KOA)before and after treatment,and to observe the effect of acupotomy on the expression of interleukin-1(IL-1)in serum,and to explore the mechanism of acupotomy in the treatment of KOA,so as to provide theoretical basis for the treatment of KOA by acupotomy.Methods: 60 KOA patients who met the inclusion criteria were randomly divided into needle knife group and Western medicine group,30 cases in each group.The needle-knife group received needle-knife treatment once a week for 3 weeks.In the western medicine group,diclofenac sodium double-release enteric-coated capsules were orally administered 75 mg once a day for 3 weeks.Before and after treatment,the scores of NRS,symptoms and signs,and serum IL-1 in KOA patients were evaluated twice,and the data between the two groups and before and after treatment were analyzed by SPSS 17.0 software.Results:1.Cure,marked effect,effective and ineffective were 7 cases,13 cases,6 cases and 4 cases respectively.The total effective rate of 30 cases in Acupotomy group was 86.67%,and the cure rate was 66.67%.In western medicine group,4 cases,7 cases,11 cases and 8 cases were cured,marked effect,effective and ineffective,respectively.The total effective rate was 73.33%,and the cure rate was 36.67%.In addition,by statistical analysis,the total effective rate and recovery rate of acupotomy group were better than those of Western medicine group(P < 0.05).2.In addition,after treatment and before treatment,the scores of each observation index were significantly decreased in both groups(P < 0.05),and there was no significant difference in NRS scores between needle-knife group and Western medicine group(P > 0.05),while the total scores of symptoms and signs were significantly decreased in needle-knife group(P < 0.05).3.The levels of IL-1 in serum of KOA patients in the two groups after treatment were significantly lower than those before treatment(P < 0.05).At the same time,compared with the western medicine group,the content of IL-1 in the needle-knife group decreased more significantly(P < 0.05).Conclusions:Needle-knife release therapy has an ideal clinical effect in improving the symptoms of KOA disease.It can not only relieve and eliminate the pain of knee joint,but also improve and restore the activity of knee joint,and reduce the level of cytokine IL-1 in serum of KOA patients.So it shows that needle knife release therapy is an ideal method for the treatment of knee osteoarthritis.
