Objective: Assessing the incidence of acute fatigue in patients with acute ischemic stroke(AIS),and exploring the related factors of post-stroke fatigue(PSF),to provide a basis for clinical prediction of PSF and development of targeted interventions,and provide directions for further exploration of the pathogenesis of PSF.Methods: According to the inclusion and exclusion criteria,193 cases were selected as the research objects from the selected AIS patients,who are hospitalized in the Department of Neurology of the affiliated hospital of Hebei University of Engineering from December 2017 to October 2019.After 2 weeks of conventional treatment,the fatigue degree of patients was assessed according to the Fatigue Severity Rating Scale(FSS)(FSS score?4 points defined as PSF).Based on the results above,the study subjects were classified as post-stroke fatigue group(63 cases)and non-fatigue group(130 cases).The general data was gathered,along with the fasting biochemical indexes on the day after admission.Between the two groups,single factor analysis,correlation analysis and logistic regression analysis were performed to screen the independent predictors of PSF.Results:(1)A total of 193 first-time AIS patients were included in this study.The average FSS score was(3.49±1.29)points,of which 63 patients experienced fatigue,accounting for 32.64%.(2)Single factor analysis of PSF in acute phase showed: There were significant differences in the 9 factors of hypertension,diabetes,NIHSS score,homocysteine(Hcy),cystatin C(CysC),fasting plasma glucose(FPG),lipoprotein cholesterol(LDL-C),C-reactive protein(CRP),and uric acid(UA)levels(P<0.05);There was no significant difference in age,gender,stroke site,body mass index(BMI),education level,triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),and lipoprotein a [Lp(a)] levels between the two groups(P>0.05).(3)Analysis of FSS scores and Hcy,CysC,FPG,CRP,LDL-C,and UA levels in patients with AIS showed:FSS scores were positively correlated with the levels of Hcy,CysC,FPG,and CRP(r: 0.572,0.280,0.502,0.436;P<0.05);there was no correlation with LDL-C,UA levels(P>0.05).(4)Logistic regression analysis shows that: Hcy(OR=1.222,95% CI: 1.090~1.369),CysC(OR=7.509,95% CI: 1.542~36.575),FPG(OR=1.427,95% CI: 1.065~1.912),They are independent predictors of PSF in the acute phase of AIS patients(P<0.05).Conclusions:(1)PSF is common in AIS patients at acute stage and is related to hypertension,diabetes,NIHSS scores,Hcy,CysC,FPG,CRP,LDL-C,and UA levels.The above factors indicate that PSF is the result of a combination of factors;(2)The FSS score is positively correlated with the levels of Hcy,CysC,FPG,and CRP,suggesting that the higher the level of the above four factors,the heavier the fatigue;(3)The levels of Hcy,CysC,and FPG may be independent factors predicting the occurrence of PSF.
