Objective:To observe the clinical effect and safety of acupuncture method of "waking up the mind and regulating the intestines and stomach" on constipation patients after ischemic stroke. Methods:Seventy patients with constipation after ischemic stroke who meet the criteria of diagnosis and inclusion of traditional Chinese and Western medicine were randomly divided into two groups according to the ratio of 1:1,treatment group A("waking up the mind and regulating the intestines and stomach" acupuncture group),control group B(ordinary acupuncture group),each group has 35 cases. The treatment group was composed of Neiguan,Baihui,Shangxing,Yintang, laoshizhen(Zhongwan,Qihai,Tianshu,Zusanli). In the routine acupuncture group, common acupuncture was used: Tianshu,Dachangshu,Shangjuxu,Sanyinjiao,Zhigou and Zhaohai. Two groups were operated once a day,5 days a week, weekend rest,1 week for a course of treatment,a total of 4 weeks. The clinical score scale of constipation(CCS),Bristol stool character scale(BSFS),quality of life scale of constipation(PAC-QOL)and neurologic deficit scale(NIHSS)were used for statistical analysis. The symptom improvement and difference between the two groups before enrollment,4 weeks after treatment and 12 weeks after treatment were compared,so as to clarify the acupuncture method of "waking up the mind, regulating the intestines and stomach" for constipation after ischemic stroke The efficacy and safety of the patients.Results:1.Comprehensive efficacy:(Full analysis Set,FSA): The total effective rates of thegroup A and group B after 4 weeks were 94.29%(33/35),88.57%(31/35),and the more obvious rate was 77.15%(27/35)and 60.00%(21/35),respectively.After 4 weeks of treatment, there was a certain effect.According to the analysis of Ridit(P < 0.05),the difference was statistically significant. The total effective rate and the more obvious rate of group A were better than that of group B,indicating that the acupuncture method of "waking up the mind and regulating the intestines and stomach" was superior to that of group B Patients with constipation after stroke have certain therapeutic advantages.(Per-protocol Set,PPS): after 4 weeks of treatment, the total effective rates of group A and B were 93.94%(31/33),87.50%(28/32),the more significant rates were 78.79%(26/33)and 62.51%(20/32)respectively,indicating that after 4 weeks of treatment, the two groups had certain therapeutic effects.According to Ridit analysis(P<0.05),the difference was statistically significant. The total effective rate and the more significant rate of group A were better than that of group B,which also showed that for ischemic group In the patients with constipation after stroke, the acupuncture method of "waking up the mind and regulating the stomach" is obviously superior to the ordinary acupuncture group.2.Comparison of the scores of each scale in the two groups: after the analysis of Fas set and PP set: for the Cleveland Clinic Score(CCS),after 4 weeks of treatment, group A and group B had the same effect on the constipation patients after ischemic stroke(P>0.05),but the CCS score in group a decreased slightly. At the 12 week follow-up,the CCS scores of group A and group B were statistically significant compared with those after treatment(P<0.01),indicating that group A and group B had a certain long-term effect, but compared with the 12 week follow-up of the two groups, the CCS scores of group A and group B had a significant difference(P<0.01),and the scores of group a decreased significantly. Therefore, the total score of CCS in group a decreased more than that in group B after 4 and 12 weeks of treatment. For Bristol Stool form Scale(BSFS),Patient Assessment of Constipation Quality of Life Scale(PAC-QOL)and Neurological Impairment Score(NIHSS),the scores of BSFS,PAC-QOL and NIHSS in the two groups were improved after 4 and 12 weeks of treatment. After 4 weeks and 12 weeks of follow-up,the scores of each scale in group A were significantly higher than those in group B.Therefore, the effect and long-term effect of group A is better than that of group B.Conclusions:1.The acupuncture method of "waking up the mind and regulating the intestines and stomach" is superior to the common acupuncture in improving the evaluation indexes and the comprehensive curative effect in the near and long term.2.The acupuncture method of "waking up the mind and regulating the intestines and stomach" has obvious advantages for the treatment of constipation after ischemic stroke in the aspects of manual metrology and the particularity of acupoint selection.3.The acupuncture method of "waking up the mind and regulating the intestines and stomach" for the treatment of constipation after ischemic stroke has the characteristics of green economy, safety and effectiveness, which should be popularized and applied clinically.
