Knowledge-attitude-practice Investigation On The Prevention Of Venous Thromboembolism By Operating Room Nurses During Operation And Analysis Of Its Influencing Factors

Objective:The questionnaire on the knowledge,confidence and behavior of operating room nurses in preventing venous thromboembolism during operation was compiled to understand the knowledge,attitude/belief and behavior scores of operating room nurses with different demographic data in the second and third level general hospitals in Shanxi Province.Preliminary analysis was made on the factors affecting the level of knowledge,confidence and performance of VTE prevention in operating room nurses.Methods:Questionnaire compiled: based on the theory of knowledge nobuyuki mode,access to domestic foreign venous thromboembolism(venous thromboembolism,VTE)related experts published studies and related literature has issued the management of clinical practice for venous thromboembolism and the prevention and treatment of guide or expert consensus,and combined with a semi-structured interview Delphi expert consultation,preparation of operating room nurses preventive surgery in patients with venous thromboembolism in the first draft of the questionnaireA preliminary questionnaire survey was conducted on 369 operating room nurses in Shanxi Province,and statistical methods were used to screen the items in the questionnaire.Finally,the final draft of the questionnaire was formed and the reliability and validity of the questionnaire were evaluated.2.Status quo survey: the final draft of the questionnaire was used to conduct adescriptive analysis of the general and basic data of 1107 operating room nurses in 35 hospitals in Shanxi Province.The score of nurses' knowledge-trust-line was expressed in the form of mean standard deviation.T test or Analysis of Variance(ANOVA)multivariate linear regression was used to analyze and discuss the score factors affecting VTE in operation room nurses.Results:1.Questionnaire preparation and evaluation: after the expert letter consultation and the screening of statistical items after the pre-survey,the three dimensions of knowledge,attitude and behavior were finally formed,with a total of 46 items of knowledge-credit-line questionnaire.Cronbach's coefficients in the total questionnaire,knowledge questionnaire,attitude questionnaire and behavior questionnaire were 0.867,0.824,0.855 and 0.915,respectively.The retest reliability of the total questionnaire,knowledge questionnaire,attitude questionnaire and behavior questionnaire were 0.869,0.724,0.779 and 0.863,respectively.The content validity(S-CVI)of the questionnaire is0.91.The above data indicate that the reliability and validity of the questionnaire is at an ideal level.Exploratory Factor Analysis(EFA)was performed on the questionnaire,and a total of 8 common factors of the characteristic value of >1 were extracted.The contribution rate of the cumulative variance was 67.310%.2.Current survey results:(1)the average score of nurses' knowledge of preventing VTE during surgery was(14.11±2.79),and the standardized score was(68.68±12.68),with a full score of 5.41%.The mean score of intraoperative VTE prevention attitude was(35.46±3.76),and the standardized score was(78.80±8.36).The mean score of intraoperative prevention of VTE was(32.79±5.01),and the standardized score was(72.87±11.13).(2)affect the nurse in the prevention of VTE knowledge score,in turn,the main factors are: level of hospital(b=2.296),and technical titles(b=1.618),whether to understand VTE related guide(b=1.527),the department have formulated in the prevention of VTE nursing process of(b=0.914)and nurses level(b=0.753),the operating room working fixed number of year(b=0.745),number of training(b=0.667),the highest degree(b=0.561),the unit leadership of protective VTE importance(b=0.379);Affect the nurse in the prevention of VTE the attitude of the main factors,in order to score: hospital grade(b=2.676),knowledge scores(b=2.305),and technical titles(b=1.847),number of training(b=1.496),the nurse level(b=1.491),whether to understand VTE related guide(b=0.093),the department have formulated in the prevention of VTE nursing process(b=0.070),the unit leadership of protective VTE importance(b=0.048);The main factors influencing the behavioral scores of nurses in the investigation of intraoperative VTE prevention were hospital grade(b=2.339),whether the department made the nursing process of intraoperative VTE prevention(b=1.736),technical title(b=1.580),training times(b=1.291),nurse level(b=1.331),attitude score(b=0.879)and knowledge score(b=0.668).Conclusion:The knowledge level of operating room nurses in the operation room of the second and tertiary general hospital of Shanxi Province about the occurrence of VTE in patients undergoing preventive surgery is similar to that of other domestic studies,and the knowledge mastery needs to be improved.The protection attitude is relatively positive,and the intraoperative protection behavior execution rate is good,but it is still necessary to strengthen the second and tertiary general hospitals to establish and improve the management system and mechanism of intraoperative VTE protection.Increase investment in adequate physical preventive equipment;To construct a scientific and effective VTE training structure to ensure patient safety and improve the quality of operating room care.
