Knowledge-attitude-practice Investigation On The Enhanced Recovery After Surgery By Operating Room Nurses And Analysis Of Its Influencing Factors

Objective:Questionnaires of knowledge-attitude-practice of ERAS for operating room nurses with good reliability and validity were developed to provide objective tools for accurately evaluating the knowledge-attitude-practice level of ERAS for operating room nurses.The present situation of ERAS knowledge,belief and practice of operating room nurses in Grade III Grade A general hospital of Shanxi Province was investigated,and the influence of different demographic characteristics on ERAS knowledge-attitude-practice scores of operating room nurses was explored,and feasible suggestions were put forward to improve the current situation.Methods:1.Questionnaire preparation:the first draft of the Questionnaire on knowledge-attitude-practice of Operating Room Nurses in enhanced recovery after surgery was constructed through theoretical analysis method,semi-structured interview method and expert letter consultation method;Sixty operating room nurses from a Grade A hospital in Taiyuan were selected for pre-investigation,and then the items were screened through discrimination analysis and correlation analysis.The reliability and validity of 335operating room nurses in the third level general hospitals of Shanxi Province were evaluated after formal investigation,and the final questionnaire was formed.2.The present situation investigation:using convenient sampling method,according to the level of first-class comprehensive hospital in Shanxi Province were row standard survey by 512 operating room nurses,use frequency,descriptive analysis form than in the operating room nurses’general data,with mean±standard deviation to describe knowledge-attitude-practice score line,using t test,variance analysis,multiple linear regression analysis,Pearson correlation analysis to explore the operating room nurses ERAS to know the influence factors of line-letter-score and the correlation between the three.Results:1.Questionnaire preparation and evaluation:The final questionnaire of knowledge-attitude-practice of enhanced recovery after surgery for operating room nurses included 3 dimensions,a total of 43 items,10 common factors were determined by exploratory factor analysis,and the cumulative variance contribution rate was77.965%.The fitting degree of confirmatory factor analysis was generally ideal,with X~2/df=2.867,RMR=0.05,GFI=0.904,PNFI=0.791,AGFI=0.891,and NFI=0.864.The content validity of the total questionnaire was 0.83,and the content validity of each item was 0.93～1.0.The Cronbach’s alpha coefficients were 0.876,0.837,0.938 and 0.867,respectively,and the retest reliabilities were 0.987,0.758,0.841 and 0.768,respectively.2.Investigation results of current situation:(1)68.19%of the operating room nurses heard of ERAS,only 22.91%have participated in standardized training,operating room nurses enhanced recovery after surgery knowledge-attitude-practice questionnaire total score 131 points,the average score(109.20+11.70),more than 100 points(78.32%),part of the total score of 21 points of knowledge,the average score was(14.83+2.40)points,right rate was 70.61%,the attitude is part of the total score of 60 points,the average score was(52.39+7.18)points,behavior part of the total score 50 points,scored an average of 41.98(6.76mm).(2)the main factors influencing the operating room nurses ERAS of knowledge points in the order:whether learning 2018 ERAS guide(b=1.416),position(b=1.094),whether received standardized training(b=0.961),the highest degree(b=0.72),the main factors affecting the operating room nurses ERAS attitude score is:whether learning 2018ERAS guide(b=1.848),the main factors affecting the operating room nurses ERAS behavior score in the order:Title(b=2.618),whether to study 2018 ERAS Guidelines(b=2.376),Title(b=1.082).(3)Pearson correlation analysis showed that there was a positive correlation between knowledge and attitude(r=0.179,P<0.001),but no correlation between knowledge and behavior(r=0.039,P=0.382),and a positive correlation between attitude and behavior(r=0.27,P<0.001).Conclusion:1.The Questionnaire on knowledge-attitude-practice of Operating Room Nurses in enhanced recovery after surgery compiled in this study has good reliability and validity,and can be used as an evaluation tool to investigate the knowledge-attitude-practice of Operating Room Nurses in ERAS.2.Generally speaking,the operating room nurses in the tertiary first-class general hospital of Shanxi Province have an above average level of ERAS knowledge,which needs to be further improved.They have a positive attitude towards ERAS,but the implementation status of ERAS measures is not ideal,and the implementation status is inconsistent.3.The hospital administrative department policy play a leading role,strengthen the training of the operating room nurses ERAS of knowledge,especially the learning guide and consensus,at the same time,the rational allocation of higher education nursing talents,encourage and facilitate low degree nurses to continue education,and as early as possible based on evidence-based ERAS of the operating room nursing standardized implementation process,to improve the compliance of ERAS measures and the quality of operating room nursing.
