| Objective The study are to investigate the ability of general practitioner on the basic clinical skills,recognition,attention degree,master station and influential factor,and explores the effect to improve the emergency service ability of clinical doctor of basic clinical department through kinds of professional training and support work from superior hospital doctors on the model of medical united department,thus proving scientific and systemic evidence on professional training and management for family physicians of basic clinical department.Methods The research were approved through three steps.The first step: acquire the survey on the emergency service ability of general practitioner.In refer to a large amount of literature,emergency basic information questionnaire,emergency skills obtaining state questionnaire and emergency assessment ability self-reported questionnaire were established based on several experts review and revised preliminary investigation.There were 72 general practitioner,from 8 basic clinical department,accepted the survey.The second step: emergency ability intervention study.The emergency ability of general practitioner were examined before acquiring kinds of professional emergency training plan on the model of medical united department in our hospital emergency department from November of 2016 to November of 2018,there months every turn.They also acquired emergency ability assessment again when they finished those professional emergency training.The results were enrolled into statistics to assess the effect of training via the model.The third step: emergency practice of basic clinical department,the guidance and examination from the experts of superior clinical department.Half of year and one year later,the experts from superior clinical department will go to the community hospital to conduct the emergency practice and exam the general practitioner acquired training emergency ability,thus assessing the effect of training.SPSS18.0 statistical software package was used to analyze data acquired from questionnaires and examination.The paired t-test was used to compare the difference.When P<0.05,the difference is statistically significant.Results A survey on 72 clinicians in our study revealed that 41.7% of them know well about basic common emergency clinical theories and skills.In addition,8.3% was not expert on the basic common emergency clinical theories and skills.65.3% master little about related skills.26.4% of them master well on related skills.Emergency theories and skills have a connected relationship with leader attention(P=0.000),working years(P=0.000),education background(P=0.023),professional title(P=0.000),master degree(P=0.000)and age(P=0.000).Most of the basic emergency theory and clinical skills were from working in hospitals(83.3%),journal(54.2%),academic conference(38.9%).84.7% willing to participate in professional training classes,such as clinical teaching.Above all,family physicians made great progress in growth of emergency ability and skills and emergency test in clinical moulds(P<0.05).The emergency test score acquired half and one year after the professional training have significance elevated.Conclusion:1.Part of community clinicians was not expert in clinical skills and basic theory.The emergency ability and basic clinical skills have great relationship with work experience,attention degreed,education background,professional title,age and so on.Attaching more attention to the emergency skills training,encouraging them to attend academic conference,in-service education,training class,making use of regional medical source sharing platform may make progress in growth of emergency ability.2.Professional emergency training plan on the model of medical united department can make great progress in growth of emergency ability.The model of emergency training project is an effective way for community physicians to improve the ability,and worthy of reference and promotion in other regions. |
