Objective:By observing the clinical effect of Sanjie Huoluo Decoction on preventing joint stiffness after TKA,the effectiveness and security of sanjiehuoluo decoction on preventing joint stiffness after TKA was evaluated,and providing theoretical and practical basis for preventing joint stiffness after TKA.Methods:A total of 66 KOA patients admitted to the first department of bone injury in our hospital affiliated from December 2018 to December 2019 for the first unilateral TKA were randomly divided into two groups,33 in each group,both groups were operated by the same group of surgeons.Control group: Postoperative regular rehabilitation training(active training + passive training),active training: ankle pump training + quadriceps isometric contraction training 1-2days after surgery,each group for 30 times,each time for 5 seconds,8 group a day;3 to 7days in the early stage of the training base upward knee flexion and interval training,each group for 30 times,each time for 5 seconds,8 group every day,and instruct the patient to stand at the bedside properly with the aid of walking aid,and walk around the ward for a short time,mainly depending on the patient's tolerance;postoperative 8-10 days up and down the stairs on the basis of preliminary training,make sure the patint's don't feel discomfort.Passive training: CPM machine training was performed on the second day after the elastic bandages were removed,with each group lasting for 20 min,2 groups per day,After each group of training,ice packs were applied for 10 min.one course of treatment for 7 days,and stopped after one course of continuous training.Experimental group: on the basis of the control group,Sanjie Huoluo Decoction was taken 1-14 days after the operation,1 dose per day,take in morning and evening.VAS scores were monitored on befor and after surgery the1 st,3rd,7th and 14 th day,respectively.Knee swelling was measured before and on the 3rd,7th and 14 th day after surgery.HSS score was performed preoperatively,on the 7th and 14 th day and 1 month postoperatively.ROM was measured at the 7th,14 th day,1st month and 3rd month after surgery.All data were analyzed by SPSS22.0 statistical software.Results:1.General information: there was no significant difference in gender,age,BMI and surgical profile between the two groups(P >0.05),indicating comparability.2.VAS score at the latter observation time point was lower than that at the previous observation time point in both groups(P < 0.05).One day after surgery,there was no statistically significant difference in VAS scores between the two groups(P > 0.05).VAS scores of the experimental group were lower than those of the control group on the 3th,7th and 14 th day after surgery,with statistically significant difference(P < 0.05),and the most significant difference was on day 3 after surgery.3.The ROM at the latter observation time point of each postoperative observation point in both groups was superior to that at the previous observation time point(P < 0.05).Three months after surgery,there was no statistically significant difference in the diagnosis of knee stiffness between the two groups(P > 0.05),but the ROM of the test group was better than that of the control group at the 7th and 14 th day after surgery,1th month and 3rd month after surgery,and the difference was statistically significant(P < 0.05).4.In both groups,the degree of knee joint swelling at the latter observation time pointwas lower than that at the previous observation time point(P < 0.05,).Three days after the operation,there was no statistically significant difference between the two groups(P > 0.05).However,at the 7th and 14 th day after the operation,the degree of knee swelling in the experimental group was significantly lower than that in the control group,and the difference was statistically significant(P < 0.05).5.The HSS score at the latter observation time point of each postoperative observation time point in both groups was higher than that at the previous observation time point(P <0.05).at the 7th,14 th day and 1st month after the operation,the HSS score of the experimental group was better than that of the control group,and the difference was statistically significant(P < 0.05).6.The patients in both groups were followed up to 3 months after the operation,and the experimental group showed no adverse reactions such as drug allergy,liver and kidney function damage and hemoglobin reduction compared with the control group.Conclusion:Postoperative of TKA additions of Sanjie Huoluo Decoction on the basis of routine rehabilitation training can reduce the pain and swelling of the knee joint,increase the range of motion of the knee joint,improve the function of the knee joint,prevent the occurrence of postoperative knee joint adhesion and stiffness effectively,and improve the quality of life of the patient,and shows high security,it is worthy of further clinical application and promotion.
