Objective:To explore the efficacy and principle of Quyu Tongluo decoction for limb pain after artificial total knee arthroplasty(TKA) and observe the clinical effect of the decoction.Methods:A total of 40 patients and 40 knees that suffered from limb pain after receiving the first unilateral TKA in Articular Orthopedics(the fifth district), Affiliated Hospital of Shandong Traditional Chinese Medicine University for Pure Knee Osteoarthritis from August 2015 to February 2016 and met the inclusion/exclusion criteria were selected. According to the time sequence, patients were randomly divided into TCM group and control group.20 patients in each group including 20 knees. After surgery, routine pain and swelling control was given. Quyu Tongluo decoction was administered to TCM group, based on routine treatment, referring to treatment standards. Patients took a dose per day since the second day after operation, which was taken warm half an hour after meal in the morning and evening for a total of 6 days. HSSknee function scores of both groups were observed and recorded before surgery and on the 3rd and 7th day after surgery. VAS pain scores and limb swelling degrees of both groups during rehabilitation exercise and bed rest were measured respectively on the 1st, 3rd, 5th and 7th day after surgery.In the meantime, venous blood samples of both groups were extracted on an empty stomach after getting up on the 1st,3rd and 7th day after surgery.These samples were sent to our clinical laboratory to measure C reactive protein(CRP), interleukin-6(IL-6). Finally, a statistical analysis of all data above was made using SPSS19.0 and a conclusion was drawn.Results:1.HSS scores of both groups before surgery were not statistically different(P>0.05), but on the 3rd and 7th day after operation, HSS scores of both groups became statistically different compared with pre-operation(P<0.05). HSS scores of both groups on the3 rd and 7th day after surgery also indicated statistical difference(P<0.05). Through an analysis and comparison of Mean Rank, HSS scores of TCM group on the 3rd and 7th day after surgery were also higher than those of control group.2.VAS scores of both groups during rehabilitation exercise and bed rest on the first day after surgery showed no statistical difference(P>0.05). VAS scores of both groups on the 3rd, 5th and 7th day after surgery were statistically different(P<0.05). Through an analysis and comparison of Mean Rank, the pain control of TCM group on the 3rd, 5th and 7th day after surgery were better than that of control group.3.All patients had their perimeters of the center of patella, and the upper and lower 10 cm of the center measured on the first day after surgery, i.e.,before TCM group took the medicine. By comparison, the data of both groups were not statistically different(P>0.05). The data of both groups on the 3rd,5th and 7th day after surgery were statistically different(P<0.05). Through an analysis and comparison of Mean Rank, the perimeters of control group weregreater than those of TCM group.4.CRP and IL-6 of both groups were measured on the first day after surgery,i.e., before TCM group took the medicine. By comparison, the data were not statistically different(P>0.05). However, on the 3rd and 7th day after surgery, the four indicators above of both groups indicated statistical differences(P<0.05). Through an analysis and comparison of Mean Rank, the data of control group were higher than those of TCM group.Conclusion:Quyu Tongluo decoction has distinct clinical effect on limb pain and swelling pain after TKA. It can control and reduce the pain and swelling indicators effectively. Meanwhile, it has a significant positive regulation effect on the objective biochemical indicators of CRP and IL-6 in the serum of patients with swelling pain after surgery and plays a remarkable role in accelerating the postoperative rehabilitation process of patients, improving their quality of life and enhancing satisfaction with surgery, etc. It is worthy of clinical popularization.
