Objective:1.To analyse the differences of blood metabolism indexes between patients with urinary stone disease with and without metabolic syndrome,in order to preliminarily explore the possible mechanism and the risk factors of urinary stone disease with metabolic in adults in Enshi.2.To analyse the relationship between abnormal blood metabolism and stone's composition in patients with urinary stone disease.3.Through the above analysis,to give some guiding suggestions for the prevention and treatment of patients with urinary stones disease in the future clinical work.Methods:150 patients with urinary stones disease treated by urology department of Enshi Prefecture Central Hospital from December 1,2018 to September1,2019 were collected.All of these patients were hospitalized in our department.The general data of the patients(name,sex,age,history of diseases,height,weight,etc.),fasting blood biochemical examination(blood electrolyte,blood lipid,renal function,blood glucose)and partial urine routine index(urine PH),the analysis'results of stone'composition were recorded.General data of the patients(name,sex,age,past history,height,weight,etc.),fasting blood diagnostic criteria of MS in the guidelines for the prevention and treatment dyslipidemia in Chinese adults(2016 revised in 2016),patients selected in this study will be divided into two groups:the group of patients with urinary stones disease and metabolic syndrome(MS+USD)and the orther group of patients with urinary stones disease(USD).SPSS24.0 software was used to analyse the difference of blood metabolism index between the two groups.According to the analysis report of stone'composition of 150 patients,the patients were grouped according to the main components:the group of calcium oxalate monohydrate stone,the group of calcium oxalate dihydrate stone,the group of carbonated apatite,the group of magnesium hexahydrate phosphate stone and the group of anhydrous uric acid stone,then the relationship between the stone'composition and abnormal blood metabolism was analysed.Results:1.The most stone'composition of the 150 patients with urinary stone disease was calcium oxalate monohydrate combined with apatite carbonate,included 43 cases(28.7%).Among them,there were 104 mixed stones(69.3%),with the most stone of calcium oxalate monohydrate combined with carbonated apatite,included 43 cases(28.7%).And there were 46 cases(30.7%)of simple stone(stone with single component),with the most calcium oxalate monohydrate,31 cases(20.7%).On the whole,Fisher accurate probability method used to test patients of different genders found that the stone'composition of patients of different genders was not statistically significant(F=13.223,P>0.05).2.There was no statistically significant in age and sex between the groups of MS+USD and USD(P>0.05).the group of MS+USD had higher levels of in the indicators of BMI?SBP?DBP?FBG?TG?UA and lower levels in that of HDL-C?urine PH compared with the group of USD,and the difference between them was statistically significant(P<0.05),and especially,the difference in this indexes of BMI?SBP?DBP?FBG?HDL-C?UA?urine PH had distinct statistically significance(P<0.01).The other indexes were not statistically significant between the two groups(P>0.05).Constructing the two-classification Logistic regression model with the above statistically significant indicators,stepwise regression,the results showed that:BMI(OR=1.659,P<0.05),diastolic blood pressure(OR=1.084,P<0.05),fasting blood glucose(OR=1.776,P<0.05),triglycerides(OR=2.836,P<0.05),blood uric acid(OR=1.010,P<0.05)were statistically significant.3.Grouping according to the main components of stones:among the two groups of stones with calcium dihydrate and anhydrous uric acid,the incidence of hyperuricemia was significantly different(P<0.05).The incidence of hypercholesterolemia was statistically different between the two groups of stones with carbonate apatite and anhydrous uric acid(X~2=3.977,P<0.05).There was statistically difference in the incidence of low urine PH between the two groups of stones with anhydrous uric acid and carbonate apatite(X~2=3.977,P<0.05).The incidence of low urine PH was statistically different between the two groups of stones with calcium oxalate monohydrate and anhydrous uric acid(X~2=3.863,P<0.05).Differences in the incidence of abnormal metabolic indicators between the remaining groups of stones were not statistically different(P>0.05).Conclusion:1.The stone'composition of adult patients with urinary stone disease in this area is mainly mixed type,and calcium oxalate and carbonated apatite are the most common ingredients.2.Among adult patients with urinary stone disease in this region,the levels of BMI,diastolic blood pressure,fasting blood glucose,triglycerides and uric acid were higher in patients with MS+USD than in patients with USD,so they are independent risk factors for patients with USD+MS.3.In this study,stone'composition in adult patients with USD in the region is associated with abnormal body metabolism:hyperuricemia,hypercholesterolemia,low urine PH.
