Objective To analyse the features of urinary stone diseases in Changshu,and explore the relationship between different compositions of urinary stone and metabolic disturbances of urine and blood.To evaluate the significance of application of urinary stone composition analysis and metabolic evaluation in basic hospital.Methods The urinary stone compositions of 446 cases were analyzed.252 of them tested comprehensive physico-chemical compositions of urine and blood were taken as the stone group,and divided into 3 groups according to major stone compositions. 80 cases with non-calculous diseases tested the same urine and blood examinations were taken as the control group.The datas of calculous groups were compared with those of the control group.Results Among the 446 cases,the most common compositions of urinary stones was calcium oxalate,the most common type of urinary stone were mixed compositions stones which all contain of calcium element. There were more males than females,with a ratio of1.84:1. The ratio of the youth,the middle-aged and the aged was 1.33:1.05:1.The ratio of the upper and the lower urinary stones was 6.56:1.The 24 h urine volume of stone group was less than the volume of control group,the urine specific gravity and urinary infection incidence of stone group were higher than those of control group,differences above were statistically significant(p<0.05).Both calcium and sodium excretion in 24 h urine of the calcium oxalate stone group were higher than the control group,and the differences were all statistically significant(p<0.05).Compared with the control group,the uric acid stone group had lower urine p H levels,higher incidences of hyperuricuria and hyperuricemia,more uric acid excretion in 24 h urine,and higher uric acid concentration in blood,and we also found that the uric acid stone group had higher levels of blood cholesterol and triglycerides,differences above were statistically significant(p<0.05).Infective stone group had higher urine pH levels and more phosphorus excretion in 24 h urine than those of control group,and the differences were statistically significant(p<0.05).No statistically significant difference appeard in the other comparisons(P>0.05).Conclusions There were close relationships between urinary stone compositions and metabolic disturbances of urine or blood.Urinary infection, 24 hour urine volume,urine specific gravity,urine p H levels, hypercalciuria, hypernatreuria,hyperphosphaturia,hyperuricuria and hyperuricemia might play roles in the formation of related urinary stone.The application of the urinary stone composition analysis and metabolic evaluation of urine and blood in basic hospital had positive clinical significance in increasing diagnosis accuracy and treatment efficiency,and reducing the recurrence risk for the local urinary calculi disease.
