Health Management Model Of "Internet" + "Centering Pregnancy":A Feasibility Study

Background: Pregnant women faced physical and psychological changes,so they had a lot of health needs.However,the current prenatal health management model in China had some disadvantages,such as the lacking of social and psychological support,low frequency and simple content of health education,and difficulty in meeting the needs of pregnant women.Centering Pregnancy(CP),which dates from the 1990 s,could through empowerment to meet the health needs of pregnant women face,and improved the pregnancy outcome effectively in western countries.But this model was less used in China.Purpose: A feasibility study was conducted on the the management model of "Internet" + "Centering Pregnancy" to explore the feasibility of the new model,and qualitative research was conducted to explore the participants' experience and improvement methods of intervention plan.Methods:1.The study was quasi-experimental and cross-sectional study.From December2017 to October 2018,the maternal health needs scale was designed,modified by means of expert consultation and pre-survey,and analyzed by using SPSS22.0 and Amos on the internal consistency,test-retest reliability,partial reliability and structural validity of the scale.By analyzing maternal health needs,preliminary topics were formed,and the specific time and topic of each offline seminar were determined by the participants.Using convenient sampling to recruit participants in the obstetrics clinic of a hospital in Wuhan in June,2019.The intervention that combined online We Chat discussions with offline seminar was conducted.The recruitment success rates,offline seminar participation rates,and other challenges encountered during the implementation of the intervention were recorded.2.A qualitative study was conducted in November 2019 to understand participants' experiences and opinions.Semi-structured interviews were used.After obtaining the consent of the participants,the one-to-one interview was conducted.During the interview,the recording was made and the interview notes were recorded.Within 12 hours after the interview,the two participants transcribed the interview contents.NVIVO11.0 was used to manage and analyze the interview data.Results:1.Maternal Health Needs Scale was a Likert scale with a total of 23 items in 6dimensions.Each item had five options.MHNS had good reliability and validity.Thetuckers-lewis Index is 0.93 and the chi-square is 693.07.The minimum difference(chi-square average/degree of freedom)was 3.22.The Comparative Fit Index is 0.94;The Root Mean Square Error of Approximation was 0.07.Cronbach's alpha coefficient of the scale was 0.94.The test-retest reliability was 0.85.The half-fold reliability is 0.85.Of the 23 items on MHNS,the items with the highest scores were the four items in the dimension of the baby's physiological safety needs.The item with the lowest score was "be able to get help and support from relevant social institutions".2.A total of 12 people were recruited for the feasibility study,with a recruitment rate of 63.16%.4 offline seminars were held,with a attendance rate of 43.75%.The feasibility study was divided into two phases according to the change of the intervention program.Stage 1: Pregnant women discussed freely in the We Chat group and regularly participate in offline seminars.The participation rate of the seminar is38.89%.Stage 2: Reduced the number of offline discussions.Pregnant women could discuss freely in the We Chat group,and at the same time,increased the push service of We Chat Official Account.The participation rate of the seminar in the second stage was 58.33%,and the topics related to pregnancy were pushed on We Chat Official Account for 7 times,with an average reading of 758.14(±1047.16)times.3.In the qualitative study,participants believed that the new model could empower participants and provide psychological and social support,but there were still some problems in the timing and introduction of the seminar topics,and participants also put forward suggestions on the participation rate.Conclusion: The "Internet" + "Centering Pregnancy" management model still retained some advantages of CP: To improve pregnant women's satisfaction,provide social and psychological support,etc.Other convenient advantages of the new pregnancy management model were needed to explore in the next pilot study.After modifying the intervention program of this feasibility study,a new intervention program was formed based on the results of the feasibility study.The intervention would be implemented in the next pilot study to verify its logic and feasibility.
