Objective:In combination with the TMSC model and suggestions of experts,basing on the investigation on the current situation and influencing factors of maternal and infant health literacy of pregnant women,the intervention program was established by combining the TMSC model and expert Suggestions the intervention program was established,to explore the effects of family-centered centering pregnancy model on maternal outcomes,psychological status,satisfaction,breastfeeding rate and maternal and infant health literacy.Methods:1.The general information and maternal and infant health literacy level of 360 subjects were investigated by using the questionnaire on general information of pregnant women and the questionnaire on maternal and infant health literacy evaluation,to discuss the current situation and influencing factors of maternal and infant health literacy of pregnant women.2.Establishment of family-centered Centering Pregnancy Intervention Model.3.Using convenience sampling method,180 pregnant women who established a card in the outpatient department of a grade a maternity and child hospital from July 2018 to November 2018 and had regular delivery examination were selected as the research objects.According to the random number table method and sealing envelope technology,the research objects were divided into the intervention group and the control group,with90 cases in each.At the time of enrollment,subjects in the two groups filled in the general information questionnaire for pregnant women,maternal and child health literacyassessment questionnaire,Self-rating Anxiety Scale(SAS),and Self-rating Depression Scale(SDS).Control group after the maternity clinic for regular check for routine health education in pregnant women school,intervention group on the basis of routine prenatal within the prescribed time to accept family-centered Centering Pregnancy Model,collected by members of the researchers study the delivery end,psychological state,the adherence of,breastfeeding rate,maternal and child health literacy scores,satisfaction,and other data and statistical analysis,compared with conventional care,family-centered Centering Pregnancy Model for pregnant women during pregnancy childbirth outcome,psychological state,satisfaction,the influence of the breastfeeding rate,etc.Results:1.Research on the status quo and influencing factors of maternal and infant health literacy of pregnant women(1)Current status of maternal and child health literacy of research subjects:The maternal and child health literacy scores of the subjects were(45.29±13.89),and each dimension was(31.99± 10.09),(8.27± 3.63),and(5.03± 2.17).(2)Maternal and child health literacy level:The subjects of the study were 15.56% with the basic health literacy of mother and child,and the passing rate of the dimensions of "basic knowledge and philosophy","healthy lifestyle and behavior",and "basic skills" were 26.67%,21.11% and 13.89%,respectively.(3)Single factor analysis of maternal and infant health literacy:In the individual basic conditions that affect the maternal and infant health literacy scores of the research subjects,the variables with statistical significance are: education level,household registration,adverse pregnancy history,whether they have had the pre-marital examination when they get married,whether they have followed the doctor's advice to have the pre-natal examination during pregnancy,self-health evaluation status,and whether they have attended the school courses for pregnant women.In the family basic conditions that affect the maternal and infant health literacy scores of the researchsubjects,the variables with statistical significance are: whether they live with their parents during pregnancy and whether they invest in family health in a year.(4)Multiple linear regression of the factors directly influencing maternal and infant health literacy scores of the study subjects:Eventually entered the regression equation of the independent variables have cultural degree,pregnancy is your doctor evaluate prenatal,self health status,whether in pregnant women school curriculum,the family in health investment situation,the regression equation is: maternal and child health literacy score = 53.33 +(-0.07)culture degree +(0.32)pregnancy check+(0.08)the situation of self health +(-9.26)in pregnant women school + 1.27 family in health investment.2.Applied research of family-centered Centering Pregnancy Model for Maternal Outcomes.(1)General information of the two groups of subjects:In this study,90 cases were included in the intervention group and 90 cases in the control group according to strict inclusion and exclusion criteria.During the implementation of the study,1 case(1.11%)was lost to follow-up in the intervention group,and 2 cases(2.22%)were lost to follow-up in the control group.Baseline data were compared between the two groups of subjects who finally completed the study,including the age,gestational age,number of pregnancies,number of births,education background and occupation of the subjects.The results showed that there was no statistically significant difference between the intervention group and the control group in baseline data(P >0.05),which was balanced and comparable.(2)The outcomes of childbirth of the two groups of subjects:The rate of transvaginal delivery and free body position application rate in the intervention group were higher than those in the control group.The incidence of perineal injury and neonatal body weight were lower than those of the control group,and the difference was statistically significant(P < 0.05).There was no significant difference between the two groups in postpartum blood loss and Apgar score 1 min(P > 0.05).(3)Feeding status,compliance of antenatal examinations and satisfaction of subjects inthe two groups:The rate of pure breast feeding,compliance of antenatal examinations and satisfaction was significantly higher in the intervention group than in the control group(P < 0.05).(4)Two groups of subjects' mental states:When they just joined,two groups of pregnant women in score of anxiety and depression had not statistical difference(P > 0.05),and after the intervention of two groups of pregnant women in labor,the differences statistically significant(P < 0.05),the score of intervention group scored lower than the control group,and intervention group after the intervention below before the intervention,the difference was statistically significant(P< 0.05)?(5)Maternal and infant health literacy scores of the two groups:In group into two groups of maternal and infant health literacy scoring when no statistical difference(P > 0.05),both groups after 24 hours postpartum maternal and infant health literacy scores compared with statistically significant difference(P < 0.05),intervention group scored higher than the control group and intervention group scored higher than into the team to score after the intervention,the difference was statistically significant(P < 0.05),while the control group after the intervention into the team to score is higher,but there was no statistically significant difference(P > 0.05).Conclusion:1.The overall level of maternal and infant health literacy of pregnant women is low,and the "basic knowledge and concept" dimension is higher than the "healthy lifestyle and behavior" dimension and the "basic skills" dimension.2.The main factors affecting maternal and infant health literacy of pregnant women include education level,whether to follow the doctor's advice during pregnancy,self-evaluation of health status,whether to participate in school courses for pregnant women,and the annual health investment of the family.3.The family-centered Centering Pregnancy Model can improve maternal and infant health literacy of pregnant women and improve their self-care ability,so as to reduce therate of cesarean delivery on maternal request,the incidence of macrosomia and perineal injury,improve the outcome of childbirth and increase the rate of breast-feeding.At the same time,this mode can improve the mental state of pregnant women,alleviate anxiety and depression,and improve their satisfaction with the delivery experience.
