Clinical Observation Of Self-made Qingre Huatan Quyu Decoction In The Treatment Of Phlegm-heat And Lung Stasis Syndrome In Acute Exacerbation Of Chronic Obstructive Pulmonary Disease

Objective:To observe the clinical efficacy of self-made Qingre Huatan Quyu decoction combined with western medicine in the treatment of(AECOPD)patients with acute exacerbation of chronic obstructive pulmonary disease of phlegm-heat and blood stasis syndrome and its effect on white blood cell(WBC),neutrophil to lymphocyte ratio(NLR)and c-reactive protein(CRP).To evaluate the effect of self-made Qingre Huatan Quyu decoction on improving symptoms and synergistic anti-infection.To provide theoretical basis for clinical treatment of acute exacerbation of chronic obstructive pulmonary disease.Methods:64 patients with phlegm-heat and lung stasis syndrome in acute exacerbation of chronic obstructive pulmonary disease were randomly divided into experimental group(n=32)and control group(n=32).The control group was treated with routine western medicine according to the GOLD guidelines(2019),while the experimental group was treated with self-made Qingre Huatan Quyu decoction on the basis of routine treatment of western medicine for 2 weeks.Mainly observe the changes of TCM syndrome score,WBC,NLR and CRP in the two groups before and after treatment.SPSS23.0 was used to analyze the data statistically.Results:During the study period,1 case was lost in the experimental group and 2 cases were excluded in the control group,so 61 patients completed the treatment.1.There was no significant difference in the general data(sex,age,course of disease,pulmonary function classification,smoking history,age distribution)between the two groups before treatment,and there was no significant difference in the total score of TCM syndrome,the score of single syndrome of TCM,WBC,NLR and CRP between the two groups before treatment(P>0.05).2.The curative effect of TCM syndrome:the total effective rate of the control group was83.33%,and that of the experimental group was 90.32%.The total effective rate of the experimental group was better than that of the control group,and the difference was statistically significant(P<0.05).3.The total score of TCM syndrome:the total score of TCM syndrome in the two groups before and after treatment was lower than that before treatment,and there was significant difference(P<0.01).Compared with the difference between the two groups before and after treatment,the decrease of total score in the experimental group was better than that in the control group,and the difference was statistically significant(P<0.01).4.TCM single syndrome score: there were significant differences between the two groups in cough,sputum quantity,wheezing,chest tightness,thirst,dry stool,fever,purple lips,lung wheezing and so on(P<0.01).There was no significant difference in lung wheezing,lip cyanosis and fever between the two groups before and after treatment(P>0.05),but in cough,sputum volume,wheezing,chest tightness,thirst and dry stool,the improvement in the experimental group was significantly better than that in the control group(P<0.05).The improvement of sputum quality in the experimental group was significantly better than that in the control group(P<0.01).5.Inflammatory indexes: the levels of WBC,NLR and CRP in the two groups before and after treatment were significantly lower than those before treatment(P<0.01).Comparedwith the difference between the two groups before and after treatment,the decrease of WBC,NLR and CRP in the experimental group was better than that in the control group,and the difference was statistically significant(P < 0.05).Conclusion:1.The clinical efficacy of routine western medicine combined with self-made Qingre Huatan Quyu decoction in the treatment of phlegm-heat and lung stasis syndrome in the acute exacerbation of chronic obstructive pulmonary disease is better than that of western medicine,which can better improve the clinical symptoms of patients and improve the quality of life.2.Western medicine routine treatment combined with self-made Qingre Huatan Quyu decoction in the treatment of patients with phlegm-heat and blood stasis lung syndrome in acute exacerbation of chronic obstructive pulmonary disease,the improvement of WBC,NLR and CRP inflammatory indexes was better than that of western medicine routine treatment.It is inferred that self-made Qingre Huatan Quyu decoction can reduce airway inflammation in patients with phlegm-heat and blood stasis lung syndrome in acute exacerbation of chronic obstructive pulmonary disease by inhibiting airway inflammation overexpression and cooperating with anti-infection.
