Application Of Crown Lengthening Surgery In Anterior Tooth Restoration

Objective:This study is to investigate the application of surgical crown lengthening surgery under the guidance of digital guide plate,assessing the stability of periodontal soft tissue and evaluating its effect in afterword restoration.Materials and methods:A review was made on 65 teeth of 12 adults patients(6 men,6 women)who underwent crown lengthening surgery in the anterior tooth with the consultation of prosthodontist in the affiliated stomatological hospital of Zhejiang university from March 2018 to March 2020.According to the individual options to use digital guide plate or not,they were divided into two groups: guide plate use group(experimental group)and routine operation group(control group).The periodontal index(periodontal probing depth,bleeding on probing,gingival margin position)was recorded after 3months follow-up.The PES/WES scale,the fitting between the prosthesis and the design before operation,as well as the satisfaction of patients were recorded afterrestoration.Results:After an 3?18 months follow-up and statistic records.The average postoperative crown height of experimental group was(6.26±0.91mm),and that of the control group was(5.35±0.53mm),showing a significant difference.The change of gingival margin in the experimental group was(0.47±0.4mm),which was smaller than it in the control group(0.93±0.76mm),showing a significant difference.The patients achieved a good periodontal condition after surgery,periodontal probing depth(2.08±0.49mm)and bleeding on probing(0.23±0.13),showing no significant difference from the control group.The position of gingival margin after restoration in the experimental group was basically consistent with the preoperative aesthetic design.The average score of PES/WES scale is greater than 7.The patient satisfaction of the prosthesis reached 75%.Conclusion:Aesthetic crown lengthening can provide sufficient crown exposure for the restoration,and the use of digital guide plate can help achieve significant periodontal soft tissue stability,suitable gingival edge position and the expected effect of the anterior region restoration.
