Study On The Immune Mechanism Of Modified Liangxue Xiaofeng Powder Regulating Th17/Treg Axis Of Recurrent Psoriasis(Blood-heat Syndrome)based On Fuxie Theory

Objective: Through observation of clinical indicators(PASI,DQOLS,etc.)and safety indicators to evaluate the efficacy and safety of Modified Liangxue Xiaofeng Powder in the treatment of recurrent psoriasis(blood-heat syndrome).Flow cytometry was used to observe the changes of Th17/Treg expression in peripheral blood of patients with recurrent psoriasis(blood fever syndrome)before and after treatment,and compared with healthy people.Based on the Th17/Treg balance,we summarize the possible immune mechanism of recurrent psoriasis(blood fever syndrome)and the target of the effect of Modified Liangxue Xiaofeng Powder.Methods: According to the inclusion criteria of recurrent psoriasis(blood-heat syndrome),30 patients were included in the experimental group,and 10 healthy people who met the criteria for healthy people were used as the control group.The experimental group underwent an eight-week Modified Liangxue Xiaofeng Powder intervention.Comparing the serological indexes(Th17,Treg)of the experimental group before and after treatment with the control group,and to compare the clinical observation indexes(PASI,DQOLS,etc.)and the flow test serological indexes(Th17,Treg)of the experimental group before and after treatment.Result:1.Clinical observation indicators: comparison of the experimental group after treatment: PASI,DQOLS,TCM syndrome scores were significantly different from those before treatment(P <0.01);combined with the improvement of the PASI score,it was concluded that 5 cases had significant curative effects,22 cases were effective,of which 27 cases had skin lesion improvement of PASI50.Follow-up of these 27 patients found that 77.8% of them had not relapsed within 6 months.2.Serological indicators of flow cytometry experiment:(1)Comparison before treatment in the experimental group: Th17,Treg,Th17/Treg have significant differences compared with the control group(P <0.01).(2)Comparison of experimental group after treatment: Th17,Treg and Th17/Treg were different from those before treatment(P <0.05),Th17,Th17/Treg were not different from those of control group(P> 0.05),but Treg there were differences(P <0.05).3.Correlation analysis: There was no correlation between changes in the experimental group before and after treatment and changes in clinical observation indicators(PASI,DQOLS,etc.)(P> 0.05).Conclusion: This study confirmed that Th17/Treg imbalance,high inflammatory Th17 expression,and low immunoregulatory Treg expression may be the material basis for psoriasis recurrence and blood heat syndrome in patients with recurrent psoriasis.Modified Liangxue Xiaofeng Powder,which is aimed at the core pathogenesis of recurrent psoriasis "asthenia in origin and asthenia in superficiality",can up-regulate the anti-inflammatory cell Treg and down-regulate the pro-inflammatory cell Th17,making the Th17/Treg inflammation-immune axis tend to balance to treat recurrent psoriasis.The mechanism of regulating Th17/Treg balance may be the immunological connotation of the two major treatment principles of "Fu Xie" theory of "strengthening healthy qi to eliminate pathogens".The treatment of relapsed psoriasis has a fast onset time and effectively shortened the course of disease.After 8 weeks of treatment,the symptoms of blood-heat syndrome and PASI were significantly improved.Follow-up found that it effectively prolonged the recurrence time and improved the quality of life of the patient.
