Objective:To investigate the significance of Th17/Treg imbalance in pathogenesis and recurrence of psoriasis vulgaris(PV),the frequency of Th17,Treg cells and the ratio of Th17/Treg in peripheral blood of patients with recurrent PV(blood-heat syndrome),patients with PV cured for more than one year and healthy persons were compared..At the same time,this research is aim to provides objective index for TCM typing of PV.Methods:30 patients with recurrent PV(blood-heat syndrome),10 patients with clinically cured PV and 10 healthy controls were selected.The expression of Th17,Treg cells in peripheral blood of three groups was detected by flow cytometry.At the same time,the correlation between the level of Th17,Treg,Th17/Treg and PASI scores of patients with recurrent PV(blood-heat syndrome)was analyzed.Results:1.Comparison of Th17 cells: the relative expression level of Th17 cells in the peripheral blood of patients with recurrent PV,patients with clinical recovery for more than one year and healthy persons were statistically different among the three groups: The recovery group was higher than that of the healthy group(P < 0.05),but there was no significant difference between the recurrent group and the recovery group.2.Comparison of Treg cells:the relative expression level of Treg cells in the peripheral blood of patients with recurrent PV,patients with clinical recovery for more than one year and healthy persons were statistically different among the three groups: The recurrence group was significantly different from the healthy group and the recovery group(P< 0.01,P < 0.05),but there was no significant difference between the recovery group and the healthy group.3.Comparison of Th17/Treg ratio: the ratio of Th17/Treg in peripheral blood of patients with recurrent PV,patients with PV cured for more than one year and healthy persons had statistical significance:The recurrence group was significantly different from the healthy group and the recovery group(P < 0.01,P < 0.05),and there was no significant difference between the recovery group and the healthy group.4.There was no linear correlation between the expression of Th17,Treg and Th17/Treg cells and PASI of patients with recurrent PV with blood-heat syndrome(P > 0.05).Conclusion:1.Compared with healthy controls,the expression of Th17 cells in peripheral blood of patients with recurrent PV(blood-heat syndrome)was high,while the expression of Treg cells was low,and the ratio of Th17/Treg was significantly increased,all of which were related to the production of blood-heat syndrome of PV.2.One year after the clinical recovery of PV,the expression of Treg cells in peripheral blood tended to be normal,but the expression of Th17 cells was still overexpressed.It is suggested that the dynamic expression of Th17 and Treg cells in peripheral blood of patients with PV may be used as a laboratory index to evaluate the clinical efficacy and prognosis of PV.
