Analysis Of Disease Composition And Cause Of Death Of ICU Inpatients In Shengjing Hospital From 2016 To 2019

Objectives:Add up the statistics of the disease composition and outcome of inpatients in the Intensive Care Unit(ICU)of Sheng Jing Hospital in the past three years,and analyze the relevant influencing factors of the death cases,so as to obtain the characteristics of the disease composition of the patients admitted in our hospital and the related risk factors of death,Provide a basis for improving the diagnosis and treatment and nursing capabilities of related diseases.Methods:Statistical collection of clinical data of patients admitted to ICU at Sheng Jing Hospital from October 2016 to September 2019,including general information such as the patient's medical history,gender,age,source,disease category and other clinical data and GCS score,APACHE II score,laboratory results and other clinical data data.Perform retrospective analysis on the data,collect and organize the data with Excel,and statistically analyze the data with SPSS 22.0.The counted data and measurement data are expressed as percentages(%)and “x±s”,respectively,and the ?2 test and t test are used,with P <0.05 was considered statistically significant.Factors that may affect the death of ICU patients such as age,gender,length of hospitalization,concomitant underlying diseases,mean arterial pressure,electrolyte disturbance and acid-base balance disorders,respiratory,circulatory and liver and kidney dysfunction,coagulation dysfunction,GCS score and APACHE II Scoring and other factors were analyzed by ?2 test.In the single factor analysis,meaningful variables are assigned as independent variables(X),different outcomes are assigned as dependent variables,and regression models are introduced for multi-factor Logistic analysis.Results:From October 2016 to September 2019,the ICU of Sheng Jing Hospital accumulatively treated 2717 severe patients,including 1218 male patients(44.38%)and1499 female patients(55.71%).The male to female ratio was 0.81:1;the age span was14 ~95 years old,average(51.94±19.02)years old;length of hospital stay 1~108d,median 6d;the main source of patients was the surgery department(2425 cases,89.25%).The disease composition of ICU patients is in order of pregnancy complications andcomplications(669 cases),malignant tumors(454 cases),accidental injuries(386 cases),infectious diseases(351 cases),sepsis(309 cases),stroke(220 cases),severe acute pancreatitis(72 cases),cardiovascular disease(65 cases)and others(191 cases).There are differences in the composition and distribution of major diseases of different genders and ages.Among the 2717 patients,2050(75.45%)were transferred back to the general ward,275 patients(10.12%)died,392 patients(14.43%)withdrew from hospital and the mortality rate was 10.12%.The disease composition of the dead patients was sepsis(79cases),stroke(43 cases),infectious diseases(41 cases),malignant tumors(29 cases),accidental injuries(28 cases),pregnancy complications and complications(19 cases),cardiovascular disease(12 cases),severe acute pancreatitis(11 cases)and others(13cases).Diseases with a high mortality rate are mainly sepsis(25.57%),stroke(19.55%)and cardiovascular disease(18.46%).Age,concomitant underlying diseases,mean arterial pressure,electrolyte disturbance and acid-base balance disorders,respiratory dysfunction,circulatory dysfunction,liver and kidney dysfunction,abnormal blood coagulation,GCS score and APACHE II score are the influencing factors of death in ICU patients(P <0.05).Age ?65 years,circulatory dysfunction,respiratory dysfunction,liver and kidney dysfunction,low GCS score,high APACHE II score are independent risk factors for death in ICU patients.Conclusion:As a special unit,many patients in ICU are with complicated disease and high mortality.Due to the setting of the medical unit in our hospital,the patients treated by the ICU in our hospital mainly come from the operating departments such as obstetrics and surgery,and are mainly young and middle-aged people and the elderly.Analysis of patients who died in the ICU ward of our hospital in the past three years,age,accompanying underlying disease,mean arterial pressure,electrolyte disturbance and acid-base balance disorders,circulatory dysfunction,respiratory dysfunction,liver and kidney dysfunction,coagulation dysfunction,GCS score And APACHE II scores are the influencing factors of death in ICU patients(P<0.05).Age ?65 years,circulatory dysfunction,respiratory dysfunction,liver and kidney dysfunction,low GCS score,high APACHE II score,etc.are independent risk factors for death in ICU patients.Clinically,patients with the above risk factors should be given enough attention,rational allocation of medical resources,and further improve the quality of medical care and the success rate of patient treatment.
