Objective:To investigate high risk factors of intensive care unit-acquired weakness in patients with sepsis and multiple organ dysfunction and multipleorgan dysfunction.Methods:This study retrospectively reviewed 129 patients with mechanical ventilation who were diagnosed sepsis and multiple organ dysfunction and multipleorgan dysfunction ,admitted to The second Affiliated Hospital of Kunming Medical University from January 1, 2015 to December 31,2016. The information including the general situation , the basic diseases(Hypertension, diabetes) ,BMI,protopathy diseases, the level of albumin before into the ICU, the time of use of the ventilator, the of treatment glucocorticoid ,nutritional mode, nutrition initiation time,ICU 3 days and 7 days nutritional status, myoglobin, ICU stay, hospital stay and APACHE II score was analyzed by using chi-square and binary logistic regression.Then multiple regression equations was applied for the linear regression analysis of correlations between the mean value of myoglobin and the length of hospital stay, the number of ICU days, the time of mechanical ventilation, APACHE II score.Results: Logistic regression analysis revealed that the level of albumin before into the ICU(P < 0.005), the time of use of the ventilator(P < 0.005),nutrition initiation time(P=0.03), myoglobin(P < 0.005)were independent risk factors for ICUAW. The linear regression showed that the value of myoglobin was positively correlated with APACHE II score and negatively correlated with hospital stay.Conclusion :1.The study showed that incidence rate of ICUAW in patients with sepsis and multiple organ dysfunction was 24%.So the clinical analysis of muscle function should be clinical examination of the ICU regular part. 2. Logistic regression analysis revealed that he level of albumin before into the ICU,the time of use of the ventilator,nutrition initiation time, ICU 7 days nutritional status, myoglobin were independent risk factors for ICUAW. So the active treatment of the primary disease,improving the basic nutrition state, the early (< 48h) and appropriate nutrition mode and shortening the mechanical ventilation time could reduce the incidence of ICUAW.3. The value of myoglobin was positively correlated with APACHE ? score. So it can be used as an indicator of the severity of the disease in patients with sepsis and multiple organ dysfunction.
