Objective: To investigate the value of coagulation function?platelet and biochemical indicators in the diagnosis of severe preeclampsia.Methods: From January 2014 to June 2015,25021 people were delivered in the Maternal and Child Health Hospital affiliated Anhui Medical University,including 657 patients were diagnosed as hypertension disorders of pregnancy,197 were diagnosed as preeclampsia,and 222 severe preeclampsia patients.283 cases with complete data were selected as the research object,including 142 cases in the preeclampsia group and 141 cases in the severe preeclampsia group.The baseline characteristics such as age,delivery weeks,BMI,blood pressure and the laboratory parameters of blood routine,coagulation,biochemical of two groups were compared.The conditional Logistic regression method was used to analyze the independent influence index of severe preeclampsia,the ROC curve was used to analyze the assistant diagnostic value of platelet count,serum calcium,uric acid,and cholesterol for severe preeclampsia.Results: 1.General information:There is no significant difference in the age and BMI between 2 groups(P>0.05).Termination of gestational weeks in severe preeclampsia group was less than that in preeclampsia group,The systolic and diastolic blood pressure in the severe preeclampsia group were higher than those in the preeclampsia group,the difference was statistically significant(P<0.05);2.There were no significant differences in DD,PT,APTT,TT,Fib,INR,Hb and HCT between the two groups(P>0.05).The PLT count in the severe preeclampsia group was lower than that in the preeclampsia group(P<0.05).The Ca and Alb level of severe preeclampsia group were lower than the preeclampsia group;Cr,BUN,UA,LDH,and Chol level were higher than the preeclampsia group(P<0.05).There was no significant difference in DBil?IBil?Glb?ALT?AST or TG level(P>0.05);3.The conditional logistic regression analysis showed that low platelet,low calcium,high uric acid and high cholesterol were independent factors associated with severe preeclampsia(P<0.05).The results of ROC curve analysis showed that the AUC of UA,Chol,PLT,Ca were 0.627,0.641,0.411,0.377,respectively,(P<0.05).Combined detection of UA,Chol,PLT,Ca have a higher auxiliary diagnostic value for the severe preeclampsia(AUC were 0.734,P<0.05).Conclusion: 1.Severe preeclampsia causes changes in coagulation,liver and kidney function;2.UA,Chol,PLT and Ca are independent risk factors for the severe preeclampsia.And both of them have auxiliary diagnostic value for its severity.Combining four indicators had the greatest diagnostic value.
