Objective :To investigate the predictive effect of serum uric acid levels on maternal-fetal outcomes with preeclampsia.Methods : A retrospective study was conducted on 257 pregnant women who had delivered in Shanghai Sixth People's Hospital Affiliated to Shanghai Jiaotong University From April 2014 to December 2015.According to the diagnostic criteria for preeclampsia,patients were divided into two groups: normal pregnancy group for 105 cases without pregnancy complications and study group for 152 cases with preeclampsia.According to whether complicated with severe index,patients with preeclampsia were divided into severe group(n=106)and non severe group(n=46).According to the onset gestational weeks,patients with preeclampsia were divided into early onset group(n=75)and late onset group(n=77).According to the gout standard(serum uric acid value ? 360umol/L),patients were divided into control uric acid group of 122 cases and high uric acid group of 135 cases.Analyze clinical datas :indexes of pregnant women within one month before delivery contain serum uric acid,serum creatinine,urea nitrogen,albumin,alanine aminotransferase,aspartate aminotransferase,hemoglobin,platelet count,hematocrit,two D-dimer,fibrinogen degradation(FDP),lactate dehydrogenase and 24 hour total urine protein(the control group was not detected the later two);pregnancy complications such as pleural or peritoneal effusion,liver or kidney dysfunction;perinatal health indicators such as newborn birth weight,theApgar score and the ratio of peak systolic to least diastolic flow velocity of fetal umbilical artery(S/D).Results : There was no statistical significance in hemoglobin and hematocrit among the groups(P > 0.05).However,there was statistical significance in serum uric acid,platelet,albumin,hepatorenal function indicators and fetal umbilical artery S/D among the groups(P < 0.05).In addition,high uric acid levels was more common in early onset or severe preeclampsia,and associated with more severe impairment of hepatorenal function and fetal distress.The Logistics regression analysis showed that serum uric acid might be one of risk factors for adverse maternal-fetal outcomes with preeclampsia.Furthermore,according to receiver operating characteristic curves(ROC),we found that: when the serum uric acid level was increased to about 400umol/L,there was statistical significance for the diagnosis of adverse maternal-fetal outcomes.Conclusion :The level of serum uric acid has important reference value for prediction of maternal-fetal outcomes and the opportunity of pregnancy termination.
