Purpose:Rotator cuff injury is a common sports injury disease.In developed countries,the number of doctors per year due to shoulder pain has taken up 1% of the medical resources,about 65% of these patients were diagnosed with rotator cuff injury,and40% of general population were asymptomatic.The common clinical manifestations of rotator cuff injury are pain and dysfunction.It affects their daily life,causes certain psychological and physiological stress to the patients,and the patients who lose a certain ability of daily living also bring different degrees of burden to the family.In this study,shoulder pain,muscle strength,joint motion and shoulder proprioception were used as indicators to explore the effect of physiotherapy,massage with physiotherapy,Maitland mobilization with physiotherapy and massage with Maitland mobilization with physiotherapy on 80 patients with rotator cuff injury.By observing the effects of different rehabilitation treatment methods on different indexes to screen out the best treatment plan and provide scientific basis evidence for clinical treatment.Method:80 patients with chronic rotator cuff injury were selected from a hospital of Anhui Provincial as research objects according to selection criteria.All the subjects were divided into four groups by random number generator:Physiotherapy group(AG,18 patients)Massage with physiotherapy group(BG,17 patients),Maitland mobilization with physiotherapy group(CG,18 patients)and Combined therapy group(DG,17 patients).The frequency of treatment is 3 times per week for patients,and 1 course for 4 weeks,the total of 2 courses.The muscle strength,pain,range of motion and the joint position sense were measured before and after treatment for 4weeks and 8 weeks,respectively.Result:1.Muscle strength resultIntra-group comparison showed: the muscle strength of abduction in B?C?D groups were increased at 8 weeks compared with that before treatment(P < 0.05).Muscle strength of internal rotation in CG were increased at 8 weeks compared with that at 4 weeks(P < 0.05).Muscle strength of internal rotation in DG were increased at 8 weeks compared with that at 4 weeks and than that before treatment(P < 0.05).After 4-week and 8-week therapy,muscle strength of external rotation of BG ?CG and DG had significant difference compared with that before treatment(P <0.05).The muscle strength of external rotation in CG were increased at 8 weeks compared with that at 4 weeks(P < 0.05).Between-group comparison showed: the muscle strength of abduction in BG were increased at 4 weeks compared with that in AG(P < 0.05).The muscle strength of abduction in B?C?D groups were increased at 8 weeks compared with that in AG(P< 0.05).The muscle strength of internal rotation in B?C groups were increased at 4weeks compared with that in AG(P < 0.01).The muscle strength of internal rotation in BG were increased at 4 weeks compared with that in DG(P < 0.05).Compared with AG,the muscle strength of internal rotation in BG?CG and DG were significantly increased at 8 weeks treatment(P < 0.01).The muscle strength of external rotation in BG ? CG and DG were significantly increased at 4 weeks and 8 weeks treatment compared with AG(P < 0.05).2.Pain score results:Intra-group comparison showed: the pain scores of all groups decreased at 4weeks and 8 weeks compared with those before treatment,there was statistically significant difference(P < 0.05),The pain scores after 8 weeks therapy of all groups had significant difference compared with those after 4 weeks therapy(P < 0.01).The comparison among the four groups showed no significant difference in pain score(P >0.05).3.Range of motion of shoulder joint results:(1)Intra-group comparison showed: the ROM of shoulder joint abduction of all groups increased at 4 weeks and 8 weeks compared with those before treatment,there were statistically significant differences(P < 0.05);Compared with 4 weeks therapy,scores of all groups were increased after 8 weeks therapy,there were significant differences(P < 0.05).Compared with AG,the abduction of BG were increased after 4 weeks therapy(P < 0.01).After 8 weeks treatment,the ROM of shoulder joint abduction in BG?CG and DG were significantly increased than that in AG(P < 0.01).(2)Intra-group comparison showed: the range of internal rotation scores of all groups increased at 4 weeks and 8 weeks compared with those before treatment(P< 0.05).Compared with 4 weeks therapy,range of internal rotation scores of all groups were increased after 8 weeks therapy,there were significant differences(P <0.05).There were no significant difference among four groups(P > 0.05).(3)Intra-group comparison showed: the range of external rotation scores of BG?CG and DG increased at 4 weeks compared with those before treatment(P <0.01).The range of external rotation scores of all groups increased at 8 weeks compared with those before treatment(P < 0.05).compared with AG,range of external rotation scores of other three groups were significantly increased at 4 weeks and 8 weeks treatment(P < 0.01).4.Shoulder proprioception results:Intra-group comparison showed: At 30 degrees,shoulder proprioception scores of BG ?CG and DG were increased after 4-week and 8-week therapy,there were significant differences(P < 0.05).At 90 degrees,shoulder proprioception of CG were increased after 4-week and 8-week therapy,there were statistically significant differences(P < 0.01).At 150 degrees,compared with before therapy,shoulder proprioception of DG were increased after 4-week therapy,significant differences existed(P < 0.01).Shoulder proprioception scores of BG ? CG and DG wereincreased after 8-week therapy,there were significant differences(P < 0.05).Compared with 4-week therapy,shoulder proprioception of CG were increased after8-week therapy,significant differences existed(P < 0.05).Between-groups comparison showed: At 30 degrees,shoulder proprioception of CG and DG had significant difference with that of AG at 4-week therapy(P < 0.05).Shoulder proprioception of DG had significant difference with that of AG at 8-week therapy(P < 0.01).At 90 degrees,there were no significant difference among four groups.At 150 degrees,compared with other three groups,shoulder proprioception of DG had significantly increased at 4-week therapy(P < 0.05).Shoulder proprioception of BG?CG and DG had significant difference with that of AG at 8-week therapy(P <0.01).Conclusion:The physiotherapy,massage with physiotherapy,Maitland mobilization with physiotherapy and massage with Maitland mobilization with physiotherapy had obvious effects on decreasing pain,improving muscle strength and shoulder ROM and proprioception of the patients with rotator cuff injury.The massage with physiotherapy,Maitland mobilization with physiotherapy and combined therapy were superior to the physiotherapy in the aspect of improving the range of external rotation.The massage with physiotherapy,Maitland mobilization with physiotherapy and combined therapy were superior to the physiotherapy in the aspect of improving the shoulder proprioception.
