Patients With Chronic Diseases Under Cross-theoretical Model Self-management Group Studies

In 2018,china's first blue book on health management: china's health management and health industry development report,the report said that the current number of chronic patients in china is about 300 million,the number of deaths accounted for 85% of the total number of deaths.Among them,hypertension is not only the potential inducing factor of many chronic diseases,but also the largest proportion of chronic diseases,which has attracted wide attention.Under the strategic plan of speeding up the promotion of "healthy China 2030",the State Council has clearly put forward that it will promote the improvement of healthy lifestyle of patients with chronic diseases and establish a patient-centered self-management system in a prominent position.Relying on the professional advantages of whole-person care and overall support,medical social workers can provide more comprehensive support for the healthy lifestyle transformation of patients with chronic diseases.On the basis of paying attention to the improvement of healthy lifestyle,it is not only typical and important to explore the self-management mode of hypertension patients,but also to provide empirical reference for the overall self-management mode of chronic diseases on the basis of effectively blocking the induction of other chronic diseases.This study is based on the core element of healthy lifestyle improvement of patients,based on social work specialty,and tries to use the guidance of cross-theoretical model,taking the self-management group of patients with hypertension as an example,to carry out a group practical study of social work intervention under cross-theoretical model.This study adheres to the core concept of self-management of "patients themselves are the main body of health management ",through the intervention of group work method,taking cross-theoretical model as the theoretical basis,realizing the organic combination of group development course and cross-theoretical change stage,starting from the previous intention period,intention period,preparation period,action period and maintenance period group process,under the guidance of social workers In order to improve the self-efficacy and rational decision-making of the group members through health presentation,doctor-patient interaction,group games and guided sharing,the group members can acquire healthy behaviors in diet,exercise and so on.Combining qualitative and quantitative evaluation methods to examine service effectiveness in multiple dimensions,it is proved that group work under the guidance of cross-theoretical model can effectively help patients with chronic diseases to improve pathological condition,enhance self-efficacy,improve behavior management and improve support network.Reviewing the course of service and combining with the change stage of cross-theoretical model,we can further explore the feasible path of social work for self-management of chronic patients under cross-theoretical model.
