Chronic non-communicable diseases are one of the most important public health problems in China.Chronic diseases have the characteristics of long duration,high mortality and disability,and many complications,which cause huge economic and psychological burden on patients.China has a large population and very limited per capita medical resources.Three-quarters of the elderly over the age of 60 suffer from one or more chronic diseases,and quite a few of them are very passive in self-health management.Group work is helpful for connecting chronically ill elderly people in the community,promoting good interaction with service targets,and constructing self-health management mutual assistance groups for chronically ill elderly people.Wuxi City entered an aging society a s early as 1983.Community J is located in the center of Wuxi City.It is a typical mature acquaintance community with a high degree of aging.Nearly 30%of the elderly population is over 60 years old.80%of the elderly in the community suffer from one or more chronic diseases,including hypertension and diabetes.Based on the current situation of the chronically ill elderly population in J community and the poor self-health management,the Community Health Service Center collaborated with social workers to launch a "love before sunset" service project.The project adopted an interactive group approach to intervene in the self-health management awareness of elderly chronic disease patients And capacity improvement.The project combined the internationally used self-efficacy scale for chronically ill patients,eight Morisky drug compliance scales,etc.to conduct a survey on the needs of the intended service target,and aimed at the elderly with chronic disease,low awareness of chronic diseases,severe negative emotions,poor living habits,Issues such as low compliance with medication and weak social support are designed to carry out corresponding group activities.The project finally achieved the expected results,and the awareness and ability of group members in self-health management were improved,which also provided a reference work model for the community to continue similar activities.The project further explores the theory and practice of self-health management of chronically ill elderly people in the field of social work.While paying attention to the biomedical status of the service targets,it also pays more attention to the state of social survival,and reflects medical care and social work professional skills to reflect humane care.However,due to the limited social work experience and professional medical knowledge,more in-depth research and exploration are needed for the management of more complicated senile comorbidities,which needs to be further enriched and developed in future practical work.
