Purpose:Hospice is a care service for patients and their families at the end of the disease.It aims to improve the quality of life of patients at the end of life and their families through early identification of pain and other symptoms,correct assessment and measures to prevent and relieve pain.Take Anhui Province as an example to know the current status of the development of hospice care,investigate the death attitudes and professional knowledge of medical staff,analyze the current problems and obstacles in the development of hospice care,and learn from the beneficial experience of foreign practices.Provide a reference for accelerating the construction of a hospice care policy system and promoting the popularization of services.Method:A combination of qualitative and quantitative research is used.(1)Through the literature review method,learn relevant theoretical research methods and past research results.(2)Conduct semi-structured interviews with medical staff and related staff to understand the basic situation of medical staff's attitude and cognition of hospice care,and organize interview data based on the Grounded Theory,Analyze the influencing factors and build a theoretical model.(3)Through the questionnaire survey,investigate the death attitudes of medical staff,their awareness of hospice care and influencing factors.Data were compiled using Epi Data3.1,and descriptive analysis,single factor analysis,and multiple linear regression analysis were performed using SPSS16.0.(4)Summarize the above information,and put forward relevant countermeasures for the development of hospice care.Results:(1)On the basis of conducting semi-structured interviews with 10 interviewees,through three levels of coding,it sorted out 73 concepts and 15 categories related to the medical staff's attitude and cognition of hospice care,and refined the core categories "A model of the mechanism of the medical staff's attitude and cognitive influencing factors".(2)Survey of cognition and influencing factors of hospice care attitudes of medical staff shows that 604 samples of FATCOD-B scales were all scored 108.45 ± 9.04,which was lower than the level of Italy,Switzerland and other countries.The results of linear regression analysis showed that the departments in the sample were surgery,women,and general hospitals,which were protective factors for the attitude of hospice care of medical staff;and medical staff with internship nursing,medical assistant department,bachelor degree or above have a weak attitude towards hospice care.(3)The overall cognition of hospice care staff is relatively low,and the proportion of medical staff in public hospitals who have learned about hospice care is relatively high,and the knowledge is mainly derived from clinical practice.The main factors that medical staff do not support for the development of hospice care were worried that "the patient's family members' misunderstanding led to hospital disputes." Some medical staff have inaccurate knowledge of the beneficiaries of the hospice care,patient admission criteria,and service coverage.Conclusion:The subject's level of attitude and cognitive toward hospice care need to be improved.The development of hospice care in China is still in its infancy,and relevant policies and operating mechanisms are urgently needed to be established and improved.Improve relevant support policies such as regional planning,medical insurance system,personnel training,organizational norms,service standards,supervision and management;advocate the responsibilities of "advance care plans" in both supply and demand sides;for the poor enthusiasm of the medical staff in our country and the low awareness of hospice care,we should strengthen the death education and professional training of medical staff,and build a certification system for the hospice care profession.
