Objective 1.To explore the breastfeeding behavior in second-child pregnant women through qualitative research.2.To find the factors of influencing breastfeeding behavior in these women by analysis and Investigation.3.To provide reference for the formulation of health education guidance programs for second-born women and strategies for improving breast-feeding rate,to provide ideas for the study of influencing factors,and to lay a theoretical foundation for the construction of continuing nursing model for second-born women after discharge.Methods 1.Qualitative research: Purposive sampling method was used.The study was performed between January to March 2019.All participants who had applied for admission and gave birth in a Nanjing first class maternity and child health care hospital were provided written informed consent.When concepts likely to be answers to the research question were repeatedly elicited,data saturation was reached,the interviews ended.At last,21 second-child pregnant women were included in the research.We used semistructured interviews with participants according to the interview guide.Data were analysed by Colaizzi's analysis procedure,and the topics and conclusions were summarized.2.Quantitative research: A self-efficacy demographic questionnaire,the breastfeeding self-efficacy scale-short form(BSES-SF),the Iowa Infant Feeding Attitude Scale(IIFAS)and social support questionnaire(SSQ)and were used to survey 97 second-child pregnant women in a Nanjing first class maternity and child health care hospital,to analyse the influencing factors of these women's breastfeeding self-efficacy.Results 1.The highest score of second-child pregnant women's breastfeeding self-efficacy was 70,the lowest score was 19,the average score is 54.94±9.462.The average score of the technique subscale was 35.36±5.995,that of the intrapersonal thoughts subscale was 19.58±3.78,the level of breastfeeding self-efficacy was in medium level.2 The average score of infant feeding attitude of second-child pregnant women was 62.36±6.21.The average score of breastfeeding was 32.44±4.46,and the average score of milk feeding was 24.02±8.24,the level was mediuml.3.The total score of social support questionnaire of second-child pregnant women was 42.76±5.99,the average score of subjective support was 23.70±3.48,the average score of objective support was 10.82±3.12,and the average score of social support utilization was 8.22±1.78.The level of social support was medium.The social support of the second-child women needed to be further strengthened,consistent with the qualitative findings.4.Results Single factor analysis of breastfeeding behavior showed that there was statistical relation between past feeding methods,past difficult breastfeeding,the duration of previous breastfeeding,the current lack of milk secretion, breastfeeding self-efficacy and its intrapersonal thoughts subscale and breastfeeding self-efficacy in the second-child pregnant women(P<0.05).5.Logistic regression analysis showed that the difficulty of breastfeeding,the duration of previous breastfeeding and the current lack of milk secretion were the factors affecting maternal breastfeeding behavior(P<0.05).Conclusion Second-child pregnant women had a correct understanding of the benefits of breastfeeding,which promoted breastfeeding behavior.In the past,it was difficult for the first breastfeeding.And then it was easy to happen again during the second feeding.The breastfeeding self-efficacy ?breastfeeding attitude and social support of the second-child pregnant women were at a moderate level.The main influencing factors of breastfeeding behavior were past difficult breastfeeding?the duration of previous breastfeeding,and the current lack of milk secretion.
