BackgroundIndividual performance is the cornerstone of organizational performance.Health workers' job performance is key to the hospital medical quality.Effort-reward imbalance will cause work-related stress.And work stress will affect job performance.Compared with other industries,health care providers face heavier workload and more emotional engagement.These will lead to a greater effort-reward imbalance for medical staff.For a long time in a state of effort-reward imbalance,the work pressure of medical staffs increases.Their physical health and mental health will be influenced.Health workers may reduce working hours,attendance,and work efficiency.Public hospital reform should insist on the public welfare and mobilize health workers' enthusiasm.To improve the public welfare needs to mobilize the enthusiasm of health workers.Therefore,the research on the effect of effort-reward imbalance on the work performance of health workers is more practical for deepening the reform of public hospitalsObjectiveThe purpose of this study is to investigate the status quo of the health workers' effort-reward imbalance and work performance.To analyze the effect of effort-reward imbalance on the performance of health workers.And provide advice for improving the work performance of health workers.MethodsA public hospital in Qingdao was selected as research objects.Questionnaires and key informant interview were used to collect research data.The main contents of the questionnaire include the basic information of the medical staff,the effort-reward imbalance questionnaire and the job performance questionnaire.The basic information of medical staff is a self-designed questionnaire,and it includes gender,age,working-age and other socio-demographic characteristics.The effort-reward imbalance questionnaire and the job performance questionnaire are maturity scales.And the questionnaires have a good validity and the reliability.The effort-reward imbalance questionnaire includes effort,reward and overcommitment.Work performance questionnaire includes task performance,job dedication and interpersonal promotion.The main contents of key informant interview include the current situation and problems of health workers' effort-reward imbalance,and effect of effort-reward imbalance on the work performance of health workers.Data analysis is mainly done by Excel 2016 and SPSSv19.0.Descriptive analysis was used to describe the basic condition of the medical staffs,the status quo of the effort-reward imbalance and work performance.Variance analysis was used to analyze the effort-reward and work performance of medical staff.Correlation analysis and multi-layer regression analysis were used to explore the relationship between effort-reward and job performance.The interview audio was transcribed into words in Word,and the main result of the interviews was summed up in principal frame method for supplement the quantitative results.ResultsThe effort-reward ratio of the investigated medical staff was 0.961±0.36.And ERI rate was 41.3 percent.The effort-reward ratio of physician was 1.07±0.42,it is higher than the effort-reward ratio of nurse(0.98±0.31).ERI rate of physician was 53.1%,it is higher than ERI rate of nurse(35.2%).The effort-reward ratio of aged,longer working-age,in-staff,senior titles,longer working hours per week and high income level medical staffs are higher(P<0.05).Male physicians are higher than female physicians in effort-reward rate.Married nurses are higher than single nurses in effort-reward rateThe total score of work performance of the investigated medical staff was 64.58±8.18.The scores of nurses' work performance and each dimension are higher than that of physicians.The level of work performance of medical workers in effort-reward imbalance(effort-reward ratio>1)were lower than those in effort-reward balance(effort-reward ratio<?1).The job performance of medical staff is negatively correlated with the effort-reward ratio,and positively correlated with the rewards.From the interview,the medical staffs believed that effort-reward imbalance would have a certain impact on job performance.Esteem and overcommitment explained 12.3%of variance in physicians' job performance.Esteem and status control explained 14.0%of variance in nurses' job performance.Conclusion and suggestionsThe level of health workers' work performance is relatively high.ERI rate of health workers was 41.3%.ERI rate of physician was higher than ERI rate of nurse.The job performance of medical staffs is negatively correlated with the effort-reward ratio.The job performance of medical staffs is positively correlated with the rewards.Rewards have a greater impact on performance than effort.The work performance of physicians and nurses is influenced by esteem.The work performance of nurses is influenced by status control.The work performance of physicians is influenced by overcommitmentTo improve the health workers' work performance,the following implications can be drawn and proposed:pay attention to the medical staffs who are in effort-reward imbalance and take effective incentive measures;to reduce the workload of medical staff;to build a harmonious healthcare environment and to improve the respect of patients and their families to medical staffs;to improve the social status of nurses and change the public's understanding of nursing profession;pay attention to the physicians' overcommitment.
