ObjectiveTo assess occupational stress and job burnout status of community medical personnel in Qingpu District of Shanghai,and analyze influencing factors of assess occupational stress and job burnout.and provide reference for improving occupational stress and job burnout status of community medical personnel.MethodsBy cross-sectional studying,480 medical personnel from three community healthcare centers were enrolled as study participants.Occupational stress was measured with Effort-Reward Imbalance Questionnaire.Job burnout was assayed by Burnout Inventory-General Survey.SPSS17.0 was used for data analysis.Results1.There were 480 questionnaires distributed and 445 valid questionnaires returned(96.95%).2.The incidence rate of occupational stress among community medical personnel was 12.13% in Qingpu District of Shanghai.The mean score of 445 medical staff was(15.43 ± 4.70)for the cost dimension and was 43.38 ± 8.43 for the return dimension.The mean score of intrinsic effort dimension was 14.64 ± 3.19.The overall mean score of the payment-return imbalance index was 0.70 ± 0.31.The results of one-way analysis of variance showed that the differences of the ERI ratio of different posts and sleep times were statistically significant(P<0.05).The results of two independent sample testes analysis showed that medical personnel with chronic diseases,long working time,children and patients had high score,and the differences of the ERI ratio were statistically significant(P<0.05).The results of Chi square testes analysis showed that the risk factors of occupational stress include being male,chronic diseases,long working time,and the differences were statistically significant(P<0.05).3.Our study showed that here were a total of 328 community medical personnel with occupational burnout in Qingpu District of Shanghai.Of them,slight burnout was reported in131 personnel(29.44%),moderate burnout was reported in 92 personnel(20.67%),and the high burnout was reported in131 personnel(23.60%).According to the MBI-GS questionnaire,the score was 9.64±4.50 for the factor of emotional exhaustion,was 13.02±5.33 for the factor of passivity,and was 15.04±5.88 for the factor of professional inefficiency.The average scores of all factors in the MBI-GS questionnaire of different demographic characteristics and occupational characteristics were compared.The results showed that the mean scores of emotional exhaustion of the medical personnel who were in the position of prevention,had chronic diseases,worked ?6 hours,slept late and had patients at home were significantly higher(P<0.05).The mean score of female and shift medical staff was also significantly higher(P<0.05).Medical personnel with chronic diseases and children at home had a strong sense of professional inefficiency(P<0.05).The results of ordinal logistic regression analysis showed that preventive position,working in shifts,having children and having patients in family were the risk factors for occupational burnout(P<0.05).4.The average scores of emotional exhaustion,negative neglect and professional inefficiency of community medical personnel with occupational stress were higher than those without occupational stress(P<0.01).The dimensions of MBI-GS questionnaire(professional inefficiency,emotional exhaustion,passive neglect)were positively correlated with effort and intrinsic effort of ERI questionnaire and occupational stress level(r = 0.27-0.59,P<0.01),and negatively correlated with the reward dimension(r =-0.29-0.47,P<0.01).The results of logistic regression analysis showed that chronic disease(OR = 3.23,P<0.01),higher level occupational stress(OR =39.70,P<0.01)and higher score of intrinsic effort were the risk factors for job burnout.ConclusionThere were more severe occupational stress and job burnout status among community medical personnel in Qingpu District of Shanghai,and there is a correlation between occupational stress and job burnout.And the degree of occupational burnout in different demographic characteristic and occupational attributes was significantly different.We Should intervene the influencing factor of job burnout,such as occupational stress,to prevent job burnout occurring.
