A Clinical Study Of Huidu Yinhua Powder In Promoting The Healing Of Infectious Wounds And Its Effect On Related Cytokines

Objective:Through clinical observation,to explore the effect mechanism of Huidu Yinhua powder in promoting wound healing after operation of perianal abscess in inflammatory reaction stage and granulation growth stage and the effect of related cytokines on wound healing.Methods:60 cases of perianal abscess with low post anal deep space,aged 18-65 years,who were treated in the anorectal outpatient department of Yunnan Traditional Chinese medicine hospital,were selected as the study object.According to the method of random number table,60 cases were randomly divided into the treatment group(30cases)of Huidu Yinhua powder and the control group(30 cases)of antibiotics.Treatment group: after operation,add and subtract the Huidu Yinhua powder:Honeysuckle 30 g,Astragalus 60 g,liquorice 10 g,Polygonum cuspidatum 10 g,Chonglou 10 g,Gleditsia 10 g,Lithospermum 10 g,angelica 15 g.(decoction of traditional Chinese medicine is uniformly decocted by the Chinese pharmacy of Yunnan hospital of traditional Chinese medicine,150ml).Take it warm after breakfast,lunch and dinner,1 bag / time,3 times / day,1 dose a day,7 days as a course of treatment.External treatment: 0.9% warm salt water 1000 ml sitting bath(15 minutes,every 7 days)Once a day),every day in hospital Zilian ointment gauze strip dressing change(in hospital Zilian ointment: in hospital preparation of Yunnan Traditional Chinese medicine hospital,approval number: Dian Yao Zhi Zi(z)20082571a,specification: 10 g per box(once a day),after the patient is hospitalized for one week,the patient is discharged from the outpatient clinic for routine dressing change to wound healing.Control group: postoperative clinical antibiotic anti infection treatment(in this experimental study,Cefmetazole sodium for injection(manufacturer:Harbin Pharmaceutical Group Pharmaceutical General Factory,approval No.: gjzz h20070240,executive standard: Chinese Pharmacopoeia,2015 version,Part II,package: sodium calcium glass injection bottle,specification: 0.5g each)1g + 0.9%sodium chloride injection 250 ml was used,General intravenous drip,twice a day,5days as a course of treatment),external treatment: the same as the treatment group.The wound pain,swelling and exudation were observed 5 days after operation,the granulation tissue growth and the time of wound healing were observed 7 and 14 days after operation.14 patients were randomly selected from the two groups,and the granulation tissue immunohistochemistry was detected on the 5th day(positive rate of CD68 and MMP-9)and the 10 th day(positive rate of VEGF and b-FGF)respectively.Results:The score improvement of wound pain and swelling in the treatment group on the 5th day after the operation was better than that of the antibiotic control group(P<0.05).5 days after surgery,the wound exudation was not statistically significant between the treatment group and the control group(P>0.05).The positive rates of CD68 and mmp-9 5 days after the operation were lower in the treatment group than in the antibiotic control group(P<0.05).The positive rate of VEGF and b-fgf was compared between the two groups on the 10 th day after the operation(P<0.05).Granulation tissue growth and wound healing days on 7 and 14 days after surgery were better in the treatment group than in the control group(P< 0.05).Conclusion:1.The addition and subtraction of Huidu Yinhua powder has the functions of clearing away heat and toxin,promoting blood circulation and cooling blood,invigorating Qi,removing blood stasis and generating new energy.It can reduce the initial inflammatory reaction of the infected wound after the operation of perianal abscess,shorten the inflammatory reaction period,promote the growth of granulation tissue,and promote the healing of the infected wound.The addition and subtraction of Huidu Yinhua powder also takes into account the inflammatory reaction period and granulation growth period of wound repair,which can reduce the inflammatory reaction It is suggested that the mechanism of shortening the inflammatory reaction period may be related to CD68 and MMP-9,and the mechanism of promoting wound healing may be related to VEGF and b-FGF.2.In terms of reducing postoperative inflammatory reaction,shortening inflammatory reaction period and promoting wound healing,the clinical application of traditional Chinese medicine can fully reflect the simple and inexpensive effect of traditional Chinese medicine,reduce the use of antibiotics after perianal abscess operation,reduce drug resistance,and maintain intestinal micro dynamic balance.
