| objective:The third lumbar transverse process syndrome is common in clinical lumbar buttock pain,and generally has obvious positive reaction points at or around the third lumbar transverse process.In recent years,due to the modernization of science and technology,people go to work to learn online gradually,so that sitting for a long time has become the daily norm;With the fast pace of life and study,people’s time for rest and exercise unable to be fully guaranteed,leading to the incidence of the disease Increasing,while the age of the affected population is declining,more and more young people are suffering from the third lumbar transverse process syndrome[1].In recent years,studies have shown that needle knife is an effective method for treatment of this disease,Therefore,this clinical study used overall acupotomy lysis for the treatment of the third lumbar transverse process syndrome.The main research is the short-term and long-term clinical efficacy of this method.and the mechanism of needle knife therapy for the third lumbar transverse process syndrome,surgical design were discussed,with a view to providing a new method of needle knife surgery and the design and diagnosis of the treatment of the third lumbar transverse process syndrome.methods:The 104 patients with the third lumbar transverse process included in this clinical study were all inpatients from department of acupuncture and moxibustion of our hospital from January 2018to August 2019.The patients were numbered according to the order of hospitalization,and then randomly divided into needle knife group(the treatment group)and electric acupuncture group(the control group),with 52 cases each.In the treatment group,52patients were treated with overall acupotomy lysis treatment,among which 2 patients withdrew from the experiment due to unbearable pain in the eye of the needle after the first acupotomy.In the control group,52 patients were treated with electric acupuncture treatment,and 2 of them withdrew from the experiment because of poor clinical results and required other therapies.The treatment cycle was 1 week in both groups and followed up 1 month after discharge.The VAS scores and JOA scores of the three groups of patients before,after,and one month after treatment were recorded and compared,and the total clinical efficacy of one month after treatment was evaluated.results:1.The basic conditions of the needle knife group and the electric acupuncture group before treatment were age,disease course,gender,unilateral and bilateral disease sites,VAS score,and JOA score.After statistical analysis,all P>0.05,there was no significant difference.2.Comparison of total clinical efficacy between the needle knife group and the electric acupuncture group:In the needle knife group,there were 50 cases,21 cases were cured,16 cases showed obvious effect,9 cases were effective,4 cases were ineffective,the healing rate was 74%,and the effective rate was 90%.In the electric acupuncture group,there were 50 cases,10 cases were cured,17 cases showed obvious effect,13 cases were effective,10 cases were ineffective,the healing rate was 54%,and the effective rate was 80%.After statistical analysis,there was a significant difference between the two groups(P<0.05),indicating that both groups were effective in treating the third lumbar transverse process syndrome,but the total clinical effect of overall acupotomy lysis in the treatment group was better than that of the electric acupuncture in the control group.3.Comparison of visual analog scores between the needle knife group and the electric acupuncture group:The comparison within the group was that the VAS scores of the two groups of patients after treatment and one month after treatment were lower than those before treatment in this group,and the difference was statistically significant(P<0.05).The comparison between the groups was the VAS score of the two groups after treatment and one month after treatment.The needle knife group was lower than the electric acupuncture group,and the difference between the two groups was significant(P<0.05).The results showed that both the needle knife group and the electric acupuncture group could alleviate the pain of patients with the third lumbar transverse process syndrome,but the needle knife group is significantly better than the electric acupuncture group in reducing the pain in the short and long term.4.Comparison of japanese orthopaedic association scores between the needle knife group and the electric acupuncture group:The comparison within the group was that the JOA scores of the two groups of patients after treatment and one month after treatment were higher than those before treatment in this group,and the difference was significant(P<0.05).The comparison between groups was the JOA score of the two groups after treatment and one month after treatment.The needle knife group was higher than the electric acupuncture group.The difference between the two groups was significant(P<0.05).The results showed that both the needle knife group and the electric acupuncture group could improve the subjective symptoms and daily activity limitation in the treatment of the third lumbar transverse process syndrome,but the needle knife group was significantly better than the electric acupuncture group in the short-term and long-term improvement of subjective symptoms and daily activity limitation.conclusion:The results of the study show that overall acupotomy lysis and electric acupuncture are effective in the treatment of the third lumbar transverse process syndrome,but the overall clinical effect of overall acupotomy lysis is better than that of electric acupuncture,and it can reduce patients in the short and long term.Pain and improvement of patients’subjective symptoms and limited daily activities are better than electric acupuncture.This needle knife surgery is worth the clinical promotion. |
