ObjectiveIn recent years,the number of diabetic nephropathy in patients with type 2 diabetes is increasing,which has become a serious public health problem.The study focus on the epidemic and influencing factors of diabetic nephropathy by the cross-sectional analysis of medical records,providing scientific basis for the prevention and treatment of diabetic nephropathy.MethodsBased on the medical record data of type 2 diabetes patients from a Three-Ahospital in urology in nearly three years,the incidence of diabetic nephropathy and its influencing factors in inpatients were investigated with the the reference of the literature and clinical guidelines.The data were recorded into the database used by STATA14.0,and SPSS21.0 was utilized for descriptive analysis,single-factor and binary logistic stepwise regression analysis.Results1.The social and demographic characteristics:among the 2897 subjects in this study,the young group(ages 1 to 44),the middle-aged group(ages 45 to 59)and older group(60 and up),counted 775(26.75%),1132(39.08%),and 990(34.17%),respectively.The numberofmales and females were 1640(56.61%)and 1,257(43.39%).According to body mass index(BMI),the number of the underweight,normal,overweight and obesity were 51(1.76%),835(28.82%),1351(46.63%),and 660(22.78%)respectively.2.The disease history:there were 1238 patients(42.70%)with high blood pressure,676(23.33%)with coronary heart disease and 1132(39.07%)with a family history of diabetes.The distribution of diabetes duration was:less than 5 years,1348(46.53%);5 to 10 years,878(30.31%);11 to 15 years,371(12.80%);and more than15 years,300(10.36%).The distribution of hypertension duration was:less than 5 years,2,109(72.80%);5 to 10 years,451(15.57%);11 to 15 years,110(3.80%);and more than15 years,227(7.83%).There were 598 patients(20.64%)with diabetic nephropathy,there were 2299 patients without diabetic nephropathy,accounting for 79.36%,735(25.37%)with diabetic retinopathy and 8(0.28%)with diabetes and pregnancy.3.The single factor analysis showed that many factors were proved statistical significances(P<0.05)of distinguishing from diabetic nephropathy in patients with type 2 diabetes,including age,high blood pressure,high blood pressure,duration,duration of diabetes,coronary heart disease,diabetic pregnancies,diabetic retinopathy,fasting glucose,fasting insulin levels,fasting C peptide level,systolic blood pressure,low density lipoprotein cholesterol(hdl-c),total cholesterol,triglyceride,creatinine,uric acid,24-hour urine protein,and urine trace albumin.4.The multi-factor logistic regression analysis results indicated that duration of hypertension and diabetes duration,diabetic retinopathy,fasting glucose,fasting C peptide,systolic blood pressure,total cholesterol,creatinine,24 hours urinary protein quantitative,and trace urinary albumin could reflect the impact of the onset of diabetes nephropathy for type 2 diabetes patients(P<0.05).Conclusion1.Patients with type 2 diabetes increases the risk of diabetic nephropathy,which seriously affects patients' health and should cause great attention.2.The onset of diabetic nephropathy is derived from the interaction of multiple factors:As diabetes and hypertension progress,type 2 diabetes increases the chances of developing into diabetic nephropathy.Diabetic complications such as diabetic retinopathy may increase the risk of diabetic nephropathy.The clinical detection such as fasting blood glucose,fasting c-peptide,cholesterol,creatinine,24-hour urine protein quantification,systolic blood pressure and urinary microalbumin could affectly reflect the the risk of diabetic nephropathy.
