Clinical Observation Of Ziyin Yiqi Jiangzhuo Decoction On Diabetic Nephropathy Patients With Deficiency Of Both Qi And Yin

Objective:To explore the effect of Ziyin Yiqi Jiangzhuo Decoction on FGF-21 in the blood of nephropathy due to diabetes patients with both qi and Yin deficiency.Methods:1.30 cases in the experimental group and 30 cases in the control group were randomly divided into two groups.Both groups were given routine western medicine treatment,and all patients were asked to eat low salt,low fat,high quality,low protein and diabetes diet;the control group was given "nephritis rehabilitation tablet" on the basis of routine treatment;the experimental group was given "Ziyin Yiqi Jiangzhuo Decoction" on the basis of routine treatment;the course of treatment was 8 weeks.There was no significant difference in the general data before treatment between the two groups.2.Observe and record the relevant laboratory indexes of the two groups(experimental group and control group)before and after treatment,such as urinary microalbumin(m Alb),24-hour urinary protein quantification(24hup),liver function(alt,AST,ALB),renal function(SCR,BUN,CCR),glycosylated hemoglobin(Hb A1c%),fasting plasma glucose(FPG),triglyceride(TG),low-density lipoprotein(LDL)The changes of total cholesterol(CHOL),Urine routine(PRO),blood routine(WBC,RBC),C-reactive protein(CRP)and serum FGF-21 were compared before and after treatment.3.Observe the changes of TCM symptoms of the two groups,and analyze the therapeutic effects of the two methods by quantifying their scores.Results: 1.Comparison of results between total clinical efficacy: By comparing the total response rates in both groups after treatment,it was found that the experimental group had a significantly better efficacy than the control group(P <0.05).2.Changes in laboratory indicators: after treatment,the laboratory indexes of the two groups,such as urinary microalbumin(m Alb),24-hour urinary protein quantity(24hup),renal function(SCR,BUN,CCR),C-reactive protein(CRP),blood fat(TG,LDL,CHOL)were significantly different(P < 0.05),and compared with the control group,the improvement of these indicators in the experimental group is more obvious.3.Comparison of serum FGF-21 indicators: The serum FGF-21 levels of the two groups of patients were compared after treatment,and it was found that the serum FGF-21 levels between the two groups were different(P <0.05),and the improvement degree of the experimental group was more than that of the control group.4.Comparison of TCM symptom efficacy: By comparing the TCM symptoms of the two groups after treatment,it was found that there were differences between the two groups(P<0.05),and the improvement of symptoms in the experimental group was better than that in the control group.Conclusion: Ziyin Yiqi Jiangzhuo decoction can validly relieve the clinical symptoms of nephropathy due to diabetes patients with qi and Yin deficiency,such as swelling of the face,fatigue of the mind,shortness of breath,dryness of mouth and pharynx,heat of the hands and feet,dizziness and tinnitus,weakness of the waist and knees,frequent urination,improve microalbuminuria(m Alb),24-hour urine protein quantification(24hup),routine urination(pro),renal function(SCR,BUN,CCR),C-reactive egg White(CRP),blood lipid(TG,LDL,TCHO)can effectively reduce the concentration of FGF-21 in serum.It is suggested that Ziyin Yiqi Jiangzhuo decoction can effectively delay the progression of diabetic nephropathy,the possible mechanism is to inhibit the accumulation of lipid and the expression of inflammatory factors;at the same time,by regulating the concentration of FGF-21 in serum,improve the state of glomerular hyperfiltration,thus inhibiting renal fibrosis,slowing down the progression of chronic kidney disease caused by diabetes mellitus,and achieving the purpose of delaying the further deterioration of renal function.
