Objectives: To observe and compare and analyze the difference of therapeutic effect of tonic purging scrapping therapy and plain scraping scrapping therapy on patients with primary hypertension with hyperactivity of liver Yang.Methods: The subjects were selected from the outpatient department of traditional Chinese medicine nursing in a third-grade a hospital of traditional Chinese medicine in hunan province from March 2019 to December 2019.A total of 58 patients were initially included.After 1 case was lost and 1 case was removed,a total of 56 patients were included in the experimental group and control group.Before enrollment,all patients received routine health guidance for hypertension,mainly including lifestyle guidance and/or regular oral antihypertensive drugs.On this basis,the experimental group and the control group were treated with tonic purging scraping therapy and plain scraping method respectively.The duration of intervention in both groups was 4 weeks,2 times a week,each lasting about 90 minutes.Blood pressure,TCM syndrome score,Hamilton anxiety scale total score(HAMA)and sf-36 score were recorded before and after intervention.Excel was used to input data and SPSS21.0 software was used for statistical analysis.Measurement data were expressed in the form of mean ± standard deviation.T-test was used for inter-group comparison of normally distributed data,paired t-test was used for comparison before and after intervention,and rank sum test was used for inter-group comparison of non-normally distributed data and comparison before and after intervention.The counting data were expressed in the form of the rate and percentage,and chi-square test was used.The rank sum test is used for the chi-square test.P<0.05 was considered statistically significant in all tests.Results:(1)Comparison of blood pressure: Systolic blood pressure before and after intervention was 152.25±8.93(mm Hg)and 134.14±5.46(mm Hg),and diastolic blood pressure was 93.00±5.32(mm Hg)and 84.00±6.35(mm Hg),respectively.Systolic blood pressure before and after intervention was 151.29±7.75(mm Hg)and 140.11±8.12(mm Hg),and diastolic blood pressure was 91.14±5.45(mm Hg)and 85.79±5.20(mm Hg),respectively,in the control group.Blood pressure in both groups decreased after intervention compared with before intervention(P<0.05).The difference of SBP and DBP in the experimental group before and after intervention was greater than that in the control group,with statistically significant difference(P<0.05).(2)Comparison of TCM syndrome scores: The TCM syndrome scores of the experimental group were 27.75±4.11(points)before intervention and 16.61±3.52(points)after intervention.The scores of the control group were 27.79±3.36(points)before intervention and 19.21±2.60(points)after intervention.The scores of TCM syndrome in both groups were reduced compared with that before intervention(P<0.05).The difference of TCM syndrome scores before and after intervention in the experimental group was greater than that in the control group,with statistically significant difference(P<0.05).(3)HAMA score and rate comparison: the total HAMA score of the experimental group before intervention was 19.50±5.47 points,and 12.64±3.62 points after intervention;the score of the control group before intervention was 18.89±4.45 points,and 15.46±3.97 points after intervention;the total HAMA score of the two groups after intervention was reduced compared with that before intervention(both P<0.05).HAMA score difference before and after intervention in the experimental group was greater than that in the control group,with statistically significant difference(P<0.05).After intervention,there were 16 cases without anxiety and 12 cases with anxiety in the experimental group.In the control group,9 cases had no anxiety and 19 cases had anxiety.There was no significant difference in HAMA rate between the two groups(P>0.05).(4)Comparison of SF-36 scores: Compared with before intervention,there were statistically significant differences in scores of 7 dimensions except PF of physiological function in the experimental group after intervention(P<0.05).In the control group,there were statistically significant differences in the scores of 6 dimensions except physiological function PF and physiological function RP(P<0.05).The scores of physiological function RP,overall health GH,vitality VT,emotional function RE and mental health MH in the experimental group were higher than those in the control group before and after intervention,and the differences were statistically significant(P<0.05).(5)Comparison of clinical efficacy and adverse reactions:(1)Blood pressure efficacy: the total effective rate of the experimental group(89.29%)was higher than that of the control group(60.71%),and the difference was statistically significant(P<0.05).(2)TCM syndrome effect: the total effective rate(92.86%)of the experimental group was higher than that of the control group(64.29%),the difference was statistically significant(P<0.05).(3)No adverse reactions occurred in both groups.Conclusions:(1)Both the therapy of purging and purging and purging can reduce the blood pressure of patients with primary hypertension with hyperactivity of the liver Yang,improve the TCM syndrome,reduce the anxiety of patients,and improve the physical and mental health status and quality of life of patients.(2)The overall nursing effect of the method is better than that of the method.The effect of supplementing and relieving scraping therapy on patients' blood pressure,TCM syndromes,and quality of life(physiological function,overall health,vitality,emotional function,and mental health)is better than that of pingbuping and relieving scraping therapy.However,the HAMA anxiety score and other dimensions of life quality of both have similar improvement effects.
