| ObjectiveBased on the"brain-gut axis"theory,acupuncture is used to treat post-stroke depression(PSD)patients,and the acupuncture points of the abdomen Tianshu(ST25),Zhongwan(RN12)and the like are added through the"tongduzhiyu"acupuncture method to achieve Tongdu adjustment,The effect of regulating qi and qi.By comparing the HAMD scale,CSS scale,ADL scale score,plasma serotonin value and serum cholecystokinin-8(CCK-8)before and after treatment in PSD patients.To explore the clinical effect of acupuncture on PSD patients,to provide new ideas and methods for the treatment of PSD patients,but also to provide a certain clinical basis for verifying the mechanism of the"brain-gut axis"in post-stroke depression.MethodsIn the study,60 PSD patients were selected,which were numbered 01-60 in the order of successive hospitalization.The random number table method was used to divide the control group and the treatment group into 30 cases according to 1:1.Both groups were given basic treatment of neurovascular cerebrovascular diseases,including control of blood pressure and blood sugar,regulation of blood lipid stability plate,nutrition of cerebral nerves,improvement of cerebral circulation,etc.The control group was given oral paroxetine hydrochloride tablets on the basis of treatment,and the treatment group was given acupuncture on the basic treatment,using"Tongduzhiyu"acupuncture plus Zusanli(ST36),Tianshu(ST25)and Zhongwan(RN12).Both groups were treated for 8weeks,the control group was treated once a day,and the treatment group was treated from Monday to Saturday.Before and after treatment,the changes of HAMD,CSS,ADL scale score,plasma serotonin value and serum CCK-8 value of the two groups of patients were recorded and compared within and between groups.Before treatment,the safety indexes such as blood routine,second stool routine,liver and kidney function,and electrocardiogram were checked.After the treatment,the safety indexes of the two groups were checked again.All the measurement data were expressed as(~—X±S),and those who met the normality were tested by t test,those who did not were tested by Wilcoxon rank sum test,count data were tested by 2 test,and grade data were tested by Wilcoxon rank sum test.When P is greater than 0.05,said no statistical significance,the comparison between the two have no obvious difference,P<0.05 was statistically significant,the comparison between the two significant differences,P<0.01 was statistically significant,the comparison between the two have significant difference,in the treatment of link,the clinical adverse events recorded,summarized and evaluated.ResultsAfter the treatment,the comparative analysis between the two groups of patients is as follows:HAMD scale score:After treatment,the scale scores of patients in the control group and the treatment group were significantly decreased,and the difference was statistically significant compared with that before treatment(P<0.05),but the difference between the two groups by paired t test was not The statistical significance(P>0.05)indicates that both acupuncture and drug therapy can improve the depression of PSD patients,and the two are equally effective in improving the depression of PSD patients.CSS scale score:After treatment,the scale scores of patients in the control group and the treatment group were significantly decreased,and the difference was statistically significant compared with that before treatment(P<0.05),but there was no difference between the two groups by paired t test The statistical significance(P>0.05)indicates that both acupuncture and drug therapy can effectively promote the recovery of neurological function in PSD patients,and there is no significant difference in the treatment effect between the two.ADL scale score:After treatment,the scale scores of the patients in the control group and the treatment group were significantly decreased,and the difference was statistically significant compared with that before treatment(P<0.05),but the difference between the two groups by paired t test was not The statistical significance(P>0.05)indicates that both acupuncture and drug therapy can significantly improve the patient's ability to live and improve their quality of life,and there is basically no difference in efficacy between the two.Plasma serotonin value:After treatment,the plasma serotonin value of the two groups was significantly different from that before treatment(P<0.05);there was no significant difference between the two groups(P>0.05).It shows that with the progress of treatment,acupuncture treatment and paroxetine hydrochloride tablets improved the plasma 5-HT value significantly,but there was no significant difference between the two.Serum CCK-8 value:The serum CCK-8 value of the two groups of patients before treatment was comparable(P>0.05);after treatment,the serum CCK-8 value of the control group was not significantly different from that before treatment(P>0.05)),The serum CCK-8 value of patients in the treatment group was significantly different from that before treatment(P>0.05),and there was a significant difference between the two groups(P<0.05).It shows that with the progress of treatment,the improvement of serum CCK-8 value by acupuncture treatment is better than that of paroxetine hydrochloride tablets,and there is a significant difference between the two.During the treatment period,one patient in the control group had gastrointestinal problems such as bitter mouth and dry mouth.One patient in the treatment group had subcutaneous hematoma and one patient had halo acupuncture.After treatment,it did not affect the subject experiment.In this study,60 patients with PSD completed treatment.Before and after treatment,there were no obvious changes and abnormalities in the blood routine,second stool routine,liver and kidney function,and electrocardiogram.ConclusionBoth groups have a certain effect on the treatment of PSD patients,but compared with the control group,the patients in the treatment group have no significant difference in the increase of HAMD,CSS,ADL score,serotonin value,and the serum CCK-8 value has a significant increase It shows that acupuncture treatment is superior to oral paroxetine hydrochloride tablets in improving CCK-8.CCK-8 can activate the?-aminobutyric acid receptor in the central nervous system,increase the concentration of Ca2+in cell fluid,block Ca2+influx,and increase brain Blood supply,improve brain metabolism,protect brain neurons;antagonize the toxic effect of glutamic acid on the brain,inhibit nerve cell swelling and rupture,breakage loss,mitochondrial swelling,etc.;increase the number of nerve cell microprojections and prolong the retention time of microprojections,Reduce cell apoptosis and have neuroprotective effect[1].At the same time,the CCK system may affect depression behavior by regulating HPA axis activity and hippocampal cell proliferation.Paroxetine hydrochloride has problems of slow onset and a few patients will have gastrointestinal discomfort.The acupuncture treatment has no toxic and side effects,and the treatment effect is good.Comprehensive analysis of acupuncture treatment is better than paroxetine hydrochloride tablets. |
