Analysis On Treatment And Prognosis Of 123 Patients With Medullary Thyroid Carcinoma

Objectives:To investigate the prognostic factors and reasonable treatment for patients with medullary thyroid carcinoma(MTC)and provides reference for clinical diagnosis and treatment.Methods:The clinical information and follow-up data of 123 patients with MTC treated in the Head&Neck surgery department of Yunnan Cancer Hospital from January 1,1995 and December 31,2017 were analyzed by univariate analysis and multifactor analysis.Results:A total of 123 MTC patients were enrolled in our study,the median age was 41 years(rang,11-73 years).The 5-year and 10-year overall survival rates were 91.6%and 77.9%,respectively,for all patients.Univariate analysis showed that the age at diagnosis with MTC<40 years had a better prognosis than that of patients ≥40 years.Male,preoperative serum calcitonin level≥1000pg/mL,primary lesions size>4cm,local infiltration and not undergoing surgery were the factors of unfavorable prognosis for patients with MTC.TNM stage Ⅰ-Ⅲ of patients had a better prognosis than that of stage Ⅳ,furthermore,distant metastasis had the worst prognosis.However,there were no significant differences in survival in terms of family history with MTC,primary lesions size>lcm,mulifocality of primary lesions,location of primary lesions,the method of surgery and the method of lymph node dissection.Further study is needed on the effect of radiotherapy and targeted drug therapy on the prognosis of patients with MTC.Conclusions:For patients with MTC,the age≥≥40 years,male,primary lesions size>4cm,the clinical stage of the tumor(TNM)diagnosis is Ⅳ stage and distant metastasis were unfavorable independent factors that affecting the survival of MTC patients.Surgical treatment can help improve the prognosis of patients.
