Research On The Implementation Of Graded Diagnosis And Treatment System In Chengdu

At present,in many areas of China,there are problems of “large-scale hospitals are overcrowded,and grass-roots hospitals can be arrogant”.How to rationally allocate regional medical resources,guide the masses to order medical treatment,reduce the cost of medical treatment,and improve the efficiency of medical treatment have become hot issues in the development of medical and health care.In order to form a reasonable and efficient medical order,the whole country is exploring the implementation of the grading diagnosis and treatment system.As the first pilot city in the country and the mega-center city in the western region,Chengdu has made many achievements in exploring the implementation of the grading diagnosis and treatment system,especially the medical association.Breakthrough progress.Taking Chengdu as an example,this paper analyzes the implementation of the grading diagnosis and treatment system in the city,and draws the results and existing problems of the implementation of the grading diagnosis and treatment system in the city,and puts forward relevant suggestions for further improvement,and comprehensive grading diagnosis and treatment for other areas.The system provides reference and reference.In order to deeply understand the successful experience and existing problems of the implementation of graded diagnosis and treatment system in Chengdu,39 policy documents and relevant information were collected from three administrative departments through government websites and field visits.The background,course and measures of the implementation of graded diagnosis and treatment system in Chengdu were analyzed by using the method of literature research.140 primary medical personnel were interviewed by visiting 7 medical institutions of different grades.A questionnaire survey was conducted among 87 patients and 21 staff members and 16 patients and their families involved in graded diagnosis and treatment.Subjective feelings and evaluation of graded diagnosis and treatment system by stakeholders were investigated.Under the guidance of goal management theory and stakeholder theory,the effectiveness of graded diagnosis and treatment system in Chengdu was analyzed,and successful experience and existence were obtained.The problems and suggestions for improvement are given.The study found that Chengdu strengthens the connotation construction of medical associations,enhances the service capacity of primary medical institutions,and improves the system of signing services for family doctors.As a core hospital,the combined secondary hospitals and primary medical institutions form a consortium of responsibility sharing and benefit sharing,and initially established a new model of disease prevention and control that “standardizes diagnosis at the higher level,monitors treatment at the grassroots level,and rehabilitation management in the community”.The layout of medical resources has been optimized,the cooperation and resource sharing of medical institutions at different levels have been strengthened,the service capacity of primary medical institutions has been improved,and high-quality medical resources have been liberated to some extent from the reception of ordinary patients and the diagnosis and treatment of common diseases.However,because the implementation of the system has certain damage to the short-term interests of some people,many patients and some medical workers have low recognition of the graded diagnosis and treatment system,and the system design itself still has insufficient improvement,such as the connotation construction of the medical association.It is still not deep enough,the incentive mechanism for medical insurance policy is still not perfect,and the price adjustment of medical services has not yet been put in place.We should further change the management and operation mode of medical and health institutions,establish a benefit-sharing mechanism of medical institutions,strengthen the construction of grass-roots medical institutions and the training and assessment of grass-roots medical personnel.
