The Research On The Problems And Its Countermeasures For Grading Diagnosis And Treatment Reformation In Dongying

We always meet many difficulties and always spend a lot of money when we see a doctor.In order to solve these problems effectively and balance the irrational allocation of medical resources,our country does lots of things which includes continuing to improve the medical and health system,expanding the coverage of basic medical insurance.In the eighteen reports,it is proposed that to build a hierarchical service system is of great necessity.Grading treatment reform aims at reversing the pattern of the current unreasonable allocation of medical resources,promoting the equalization of basic medical services,controlling the excessive growth of medical expenses and achieving the goal of taking medical treatment in order.In recent years,our country has established a number of institutional norms in order to strengthen the exploration of diagnosis and treatment.A number of documents also have been published to strengthen the construction of community and rural health technology.Based on the practical experience of reform in some countries,the State Council issued guidance on promoting the construction of grading clinics system in September 2015,announced the grading and evaluation criteria for the pilot work and proposed specific requirements.The scope of reform is also expanding.Now there are nearly 20 provinces putting grading diagnosis and treatment reform into practice.In order to solve the problems such as uneven distribution of medical resources,the pressure of medical insurance fund,the medical pressure of the third grade hospital,the disorderly medical treatments,Dongying city has also carried out the reform and exploration of grading diagnosis and treatment.This paper summarizes the supporting policies,the operation of medical institutions and the construction of information system of Dongying city,and analyzes the foundation and condition of carrying out the reform of grading diagnosis and treatment.In 2016,Dongying city sets up an organization of grading medical reform,issues Opinions on Promoting the Implementation of Grading diagnosis and treatment System and the Scheme of promoting grading diagnosis and treatment of Dongying in 2016,and carries out the pilot work of grading clinics in the eastern region.The reform has made some achievements.Based on the situation of Dongying city,the paper chooses five aspects to explore,including disability,interest barriers,institutional barriers,information barriers and supervision problems.And clarifies causes of the construction of talent team,operation of medical institutions,the way of treatment and sectoral coordination.Through the summary of the experiences of the advanced cities,including family doctor signing service and regional medical associations in Shanghai,the three division of Xiamen and regional medical service community of Tianchang,Anhui.This paper analyzes experiences and enlightenments from the operating mode,policy support,quality control and information system security.Based on the above analysis,this paper points out that it is necessary to further enhance the ability of medical services,strengthen the supporting policy and the construction of information platform,improve the evaluation system which is based on the people's health of Dongying,do the work of publicity and guidance well,and achieve the purpose of promoting the city's grading treatment reform.Above all,the starting point of the reform of grading diagnosis and treatment in Dongying should provide continuity,systematic diagnosis and treatment services for the resident.Dongying city should focus on promoting the transformation of medical service model,inosculate prevention and treatment as a whole,strengthen the fine management and basic medical services,link with multiple departments,use modern information management system to smooth referral channels,improve the treatment efficiency,in order to create a comprehensive medical treatment patterns of the first treatment in the primary medical institutions,two-way referral,different treatments to acute and chronic diseases and vertical linkage.
