Study On The Effect Of Grading Diagnosis And Treatment In Chongqing Medical Association

Objective: This study takes chronic diseases(taking diabetes mellitus as an example)as the breakthrough point and analyzes the present situation of grading diagnosis and treatment in Chongqing medical association,to evaluate the effect of grading diagnosis and treatment of diabetes in the year before,during and after the establishment of the medical association by TOPSIS method and rank-sum ratio,discussing the key problems and causes which affects the implementation of grading diagnosis and treatment in the medical association and putting forward corresponding policy recommendations,hoping to provide decision-making reference for the improvement of medical association construction in Chongqing and theoretical basis for the formation of diabetes and even the entire grading diagnosis and treatment system.Method: 1.Literature research.Retrieving the literature that's related to medical association,grading diagnosis and treatment,evaluation methods of effect and so on through the database,then collating and summarizing the data.2.Questionnaire survey.Questionnaire survey was conducted among the head of the medical association office,medical staff of endocrinedepartment and diabetic patients to understand the realization of grading diagnosis and treatment of diabetes mellitus,as well as the recognition and evaluation of doctors and patients in the medical association.3.Depth interview.To conduct a depth interview with the head of the medical association office and the endocrine department and even the relevant leaders of the health administration department,to understand the overall operation of the medical association,the problems and causes,the implementation of grading diagnosis and treatment,etc.4.Expert consultation.Consulting experts in different fields such as public health,health management,health economic and so on while establishing the evaluation index system,to ensure the rationality and scientificalness of the evaluation index system.Result: 1.Based on TOPSIS method and rank-sum ratio,the comprehensive evaluation of the effect of grading diagnosis of diabetes mellitus before and after the establishment of medical association in Chongqing shows that the comprehensive evaluation score of one year after the establishment of the medical association is the highest while the score of one year before is the lowest,to a certain extent which indicates that the establishment of the medical association is conducive to the formation of grading diagnosis and treatment of diabetes mellitus.2.The effect of the medical association on the formation of diabetes mellitus grading diagnosis and treatment pattern: the service capacity of primary medical institutions has been improved,the growth rate of medical expenses has been effectively controlled,the flow of patient's medical treatment has gradually tilted to the grassroots level,and patients have better subjective perceptions of medical treatment in the medical association.3.There are still many challenges for the construction of the diabetesmellitus grading diagnosis and treatment system in the medical association:the supporting security policy needs to be improved,the focus of medical resources allocation has not sunk,the problem of structural "difficult to see a doctor" has not been alleviated,the phenomenon of "easy to turn up and difficult to turn down" is dependent,the effect of upper and lower help of the medical association is not good,the task of information construction in the medical association is arduous.Conclusion: The medical association of Chongqing has achieved initial results in promoting the formation of grading diagnosis and treatment system of diabetes mellitus,but the results achieved are at a low level and in the primary stage.It is suggested that measures should be taken to improve the construction of the medical association,such as strengthening top-level design to improve the supporting policies,optimizing the allocation of resources to enhance grass-roots capability,defining functional orientation to promote the division and cooperation,improving the information platform and strengthening supervision and assessment,so as to better promote the formation of a scientific,rational and orderly grading system of diagnosis and treatment.
