?Objective? To explore the prevalence of acne among Chinese college students, the risk factors of acne among college students, its severity, burden of disease and the socioeconomic effects,so as to provide a reference for better prevention and treatment of acne in adolescents.?Methods? Using the cluster sampling method,a total of 27,114 freshmen enrolled in 2018 of 5 comprehensive universities in China were selected, and all agreed to participate in the health check and online questionnaire survey.Among the 27,114 students,7312 freshmen from Huazhong University of Science and Technology were selected to conduct further detailed evaluation of patients with moderate to severe acne including the types of acne lesions, distribution sites and medical conditions.Data entry was performed using Epidata 3.1 software, and statistical analysis was performed using SPSS 23.0 statistical software.?Results?A total of 27,114 students were enrolled in the five universities.A total of 26,254students(97.0%)finished the medical examination,21,088 students(78.0%)finished the questionnaire and 20,138(75.0%)valid records meeting our criteria were analyzed and the total prevalence of moderate to severe acne was 10.4%.Social demography: The geographical distribution of moderate to severe acne is statistically different(P<0.001),the prevalence rate in the north is higher than the central, southern and western regions.Male predilection is identified, men /women was 2:1(a ORfemale 0.47).High BMI is associated with moderate to severe acne(a OR1.01,P<0.001),Lifestyle : The intake of cattle/sheep milk 3-6 times/week(a OR 1.131,P=0.03),the frequency of pork intake(a OR1-3d/w 1.97,P=0.019,a OR4-6d/w1.225,P=0.013)maybe two risk factors for moderate to severe acne.Spicy food(a OR Slightly spicy0.855,P=0.015,a OR Moderate spicy0.667,P<0.001,a OR Very spicy 0.531,P<0.001)maybe a protective factor for moderate to severe acne,There is no statistical different in the intake of beef and aquatic products(P>0.05);Sunscreen & skin cleaning habits: cleansing frequency 1-2 times per day(a OR1tims/w0.496,P<0.001,a OR2times/week0.802,P<0.001)maybe risk factors for moderate to severe acne, sunscreen + physical sunscreen(a OR 0.647,P<0.001),sunscreen frequency more than 3 times a week(a OR 0.649,P=0.001)maybe protective factors;female menstrual habits: menarche age(a OR0.921,P=0.009)may be associated with moderate to severe acne, the prevalence of moderate to severe acne decreased by 8% for each additional year, and there was no statistical difference in menstrual flow, duration of circulation, contraceptive use,and polycystic ovary syndrome(P>0.05).Quality of life: Acne have an impact on the quality of life of patients, and self-care ability is affected(P=0.018),but there is no statistically significant difference in anxiety and depressive symptoms in patients with or without moderate to severe acne(P>0.05).Among the 7312 freshmen enrolled in Huazhong University of Science and Technology, the prevalence of skin diseases was 50.5%,and the prevalence of moderate to severe acne was 9.0%,and the quality of life in severe acne and in female groups may be affected more than mild acne and male.?Conclusion?1.Prevalence rate: The prevalence of moderate to severe acne among Chinese college students is 10.40%.2.Risk /Protective Factors: Intake of cattle / goat milk, frequency of pork, cleansing frequency may be risk factors of moderate to severe acne.Eatting spicy, sunscreen + physical sunscreen and sunscreen frequency may be protective factors for moderate to severe acne;Acne prevalence varies geographically, males have significant disease predominance,BMI,menarche age moderate to severe acne, acne self-care ability affected.3.Diagnosis and Treatment: Acne seriously affects the face of college students, the general diagnosis and treatment of acne are not enough, the course of disease and the severity of acne have no significant correlation, severe acne have a greater impact on the quality of life of patients, and the impact is greater on women than that of men.
