Clinical Mechanism Of Double PPH In The Treatment Of Severe Mixed Hemorrhoids

Objective: To explore the clinical mechanism of double PPH in the treatment of severe mixed hemorrhoidsMethods: A total of 144 inpatients with severe mixed hemorrhoids were enrolled in the Department of Anorectal Surgery,the First Affiliated Hospital of China Medical University from December 2016 to March 2018.They were divided into double PPH group and PPH group.The gender,age,duration,symptoms,and symptoms of the two groups were There is no significant difference in general information such as staging and is comparable.Observed and evaluated the operation time,intraoperative blood loss,mucosal resection width,hospitalization time,postoperative pain score,postoperative and postoperative LONGO ODS constipation score,preoperative and long-term complication rate,and The symptoms of relapsed patients were analyzed.Results: Both groups of patients successfully completed the operation.1.The operation time and mucosal resection width of the double PPH group were larger than those of the PPH group.The difference between the two groups was statistically significant(P<0.05).There was no significant difference between the two groups in intraoperative blood loss and hospitalization time(P>0.05).2.Postoperative pain scores were compared.There was no significant difference in pain scores between the two groups on the first day,the second day after surgery,and the third day after surgery(P>0.05).3.LONGO ODS constipation scores were compared.There was no significant difference in ODS scores between the two groups before operation,1 month after operation,6months after operation and 12 months after operation(P>0.05).4,early complications,both groups of patients with acute bleeding,acute urinary retention after surgery,but the early complications were not statistically significant(P>0.05),no rectal vaginal leakage in both groups.5.Long-term complications were compared.There were no significant differences in anastomotic inflammation,anastomotic rupture,and anastomotic stenosis between the two groups(P>0.05).There were 3 cases of anal bulge in the double PPH group.PPH group 13 cases were statistically significant(P<0.05).No fecal incontinence occurred in either group.6.There were 7 patients with recurrence in the two groups,including 1 in the double PPH group and 6 in the PPH group.There was no significant difference between the two groups(P>0.05).7.Further statistical analysis was performed on the symptoms of relapsed patients.One patient with recurrent blood in the double PPH group and four patients with recurrent blood in the PPH group,including 3 patients in the PPH1 group and 1 in the PPH2 group.There was no statistical significance(P>0.05).There were no recurrences in patients with double PPH recurrence,and 5 patients with recurrence in PPH group,including 4 in PPH1 group and 1 in PPH2 group.The difference between the three groups was statistically significant(P<0.05).Conclusions: The clinical mechanism of double PPH in the treatment of severe mixed hemorrhoids can achieve a better therapeutic effect in the treatment of severe mixed hemorrhoids combined with rectal mucosal prolapse.
