Study On Pulmonary Arterial Pressure And Serum Uric Acid In Patients With Chronic Obstructive Pulmonary Disease In Plateau Area

Objective:The purpose of this study was to mensurate whether there were differences in the levels of SUA,SUA/Cr,PaO2,SaO2,PaCO2,and PASP in patients with chronic obstructive pulmonary disease in plateau and plain areas,determine whether there is a correlation between SUA,SUA/Cr and PaO2,SaO2,PaCO2 and PASP in patients with chronic obstructive pulmonary disease in plateau areas,explore the significance of SUA in the evaluation of patients with chronic obstructive pulmonary disease,and provide evidence-based medical basis for early treatment and early intervention.Methods:From November 2017 to October 2018,30 cases of AECOPD and 30 cases of AECOPD-PH in plateau area(above 2500m above sea level)were randomly selected,in the same period,30 cases of AECOPD and 30 cases of AECOPD-PH in plain area(under 500m above sea level)were randomly selected as the control group.Statistical analysis was performed on the research indexes of SUA,SUA/Cr,PaO2,SaO2,PaCO2 and PASP.Results:1.SUA and SUA/Cr of patients with AECOPD-PH in the plateau group and the plain group were higher than those patients with AECOPD(P<0.05);The SUA and SUA/Cr of patients with AECOPD and AECOPD-PH in the plateau group were higher than the corresponding plain group.(P<0.05).2.PaO2 and SaO2 of patients with AECOPD-PH in plateau group and plain group were lower than those patients with AECOPD,and PASP was higher than the patients with AECOPD(P<0.05).3.PaO2,SaO2 and PaCO2 of patients with AECOPD and AECOPD-PH in the plateau group were lower than those of corresponding plain group,and PASP was higher than that of corresponding plain group(P<0.05).PaCO2 of patients with AECOPD-PH in the plain group was higher than those patients with AECOPD(P<0.05),while PaCO2 of no significant difference between patients with AECOPD and AECOPD-PH in the plateau group(P<0.05).4.SUA and SUA/Cr of patients with AECOPD and AECOPD-PH in the plateau group were negatively correlated with PaO2 and SaO2,positively correlated with PASP,and not correlated with PaCO2.Conclusion:1.The level of SUA and SUA/Cr were involved in the formation of pulmonary hypertension under the condition of chronic hypoxia,and the increase of SUA and SUA/Cr had some certain prediction and evaluation effect on pulmonary hypertension.2.SUA as an antioxidant was elevated under oxidative stress,more suggesting that SUA participates in the oxidation/antioxidation mechanism of AECOPD.3.Hypoxia was more serious in patients with chronic obstructive pulmonary disease combined with pulmonary hypertension than in patients with chronic obstructive pulmonary disease alone;The hypoxia in patients with chronic obstructive pulmonary disease and chronic obstructive pulmonary disease combined with pulmonary hypertension in the plateau area was more obvious than those in the plain area,and the pulmonary systolic pressure was higher than those in the plain area,which further confirmed that hypoxia was the main mechanism involved in the formation of pulmonary hypertension.
