Objective To explore the effect of smoking on pulmonary artery pressure in patients with chronic obstructive pulmonary disease(COPD),and to provide reference for prevention,diagnosis and treatment of COPD with pulmonary hypertension.Methods 120 patients who were hospitalized in Minda Hospital Affiliated to Hubei Minzu Universty from January 1,2015 to December 31,2019 with complete case data and the first diagnosis of COPD were selected as the study objects.According to whether to smoke or not and whether to quit smoking,they were divided into non-smoking group,smoking group and quitting group.The basic information(including name,gender,age,occupation,etc.)of the case data were collected.The systolic pressure(PASP),pulmonary artery width(PAW),anteroposterior diameter of right were measured by echocardiography Venture,RVD,D-dimer,blood routine: hemoglobin(HB),hematocrit(HCT),blood gas analysis without oxygen: arterial partial pressure of oxygen(PaO2),arterial carbon dioxide partial pressure(PaCO2),actual bicarbonate Biocarbonate,ab),coagulation function: plasma prothrombin time(PT),activated partial prothrombin time(APTT),thrombin time(TT),fibrinogen time(FIB)and other indicators,for statistical analysis,to screen items with statistical significance.Results there was no significant difference in age,gender,occupation and TT among the three groups(P > 0.05);The difference of Hb,HCT,PaCO2 and ab between smoking group and non-smoking group was statistically significant(P < 0.05),but there was no statistical significance between smoking group and smoking group and non-smoking group(P > 0.05);the difference of APTT,FIB,Pt,D-D,paw and RVD between smoking group,smoking group and non-smoking group was statistically significant(P < 0.05),but there was no statistical significance between smoking group and smoking group There were significant differences in PaO2 and PASP among the three groups(P < 0.05),among which PaO2 in the smoking group < smoking group < non-smoking group,PSAP in the non-smoking group < smoking group < smoking group;the correlation between PASP and each index was found and PASP,PaO2,D-D and smoking group were found There was a positive correlation between the smoking index of.Conclusion:(1)Patients with COPD who smoke are more likely to have hypoxic hypercapnia and more severe hypoxia.(2)Smoking patients with COPD are prone to D-dimer elevation and coagulation disorder,with a higher risk of thrombosis.Quitting smoking in time can reduce the pulmonary artery pressure in COPD patients.(3)Smoking can directly or indirectly lead to the increase of pulmonary artery pressure in COPD patients.Quitting smoking in time is helpful to reduce pulmonary artery pressure.
