Objective:By observing the levels of hypoxia-inducible factor-1??HIF-1??and serum uric acid in patients with COPD-related pulmonary hypertension,it is to discuss the role of HIF-1?and serum uric acid in the pathogenesis and severity value of COPD-related PH patients.Methods:The samples are sixty patients with COPD who were admitted to the Department of Respiratory Medicine of Hebei General Hospital from June 2018 to January 2019.Based on the results of pulmonary artery systolic pressure?PASP?measured by echocardiography,it was divided into group COPD?30 cases?,and COPD combined with Group PH?COPD-PH group??30 cases?;Selected 25 healthy subjects who underwent physical examination in our hospital as the healthy control group.Extracted fasting venous blood from all subjects,and detected their serum HIF-1?level by ELISA.This study brought creatinine and meanwhile assessed the value of serum uric acid in diseases by uric acid/creatinine to mitigate the effect of renal excretion rate on the outcomes.All the enrolled patients completed blood gas analysis and echocardiography after admission.The data obtained in this study were statistically analyzed using SPSS 21.0 For Windows software package.The metering data were expressed as mean±standard deviation??x±s?.The two groups were compared by t test.The comparison of multiple groups was analyzed by one-way ANOVA,with Spearman method for correlation analysis,and chi-square test for counting data.P<0.05 has great significance on statistics.Results:1.The total number of the enrolled samples in the COPD group is 30,including 25 males and 5 female.There were 30 samples in the COPD-PH group,including 27 males and 3 females.While,in the healthy control group,there were 25 samples,including 18 males and 7 females.There were no significant differences in gender composition,age,smoking index and body mass index between the three groups?P>0.05?.2.Comparison of the three groups:HIF-1?,uric acid and uric acid/creatinine levels in the group COPD and group COPD-PH were higher than those in the healthy control group,with statistically significant differences?P<0.05?.While,HIF-1?,serum uric acid and uric acid/creatinine levels in the group COPD-PH were higher than those in the group COPD,and the difference was statistically significant?P<0.05?.3.The comparison between group COPD and group COPD-PH:the PaO2 level in group COPD-PH was lower than that in group COPD,and the difference was statistically significant?P<0.05?.The levels of PaCO2,PASP and transverse diameter of right ventricular in group COPD-PH were higher than those in group COPD,and the differences were statistically significant?P<0.05?.4.Correlation analysis of various indicators in group COPD-PH:HIF-1?was positively correlated with PASP,transverse diameter of right ventricular and PaCO2?r=0.596,r=0.514,r=0.576,P<0.05?,and HIF-1?was negatively correlated with PaO2?r=-0.439,P<0.05?.Both have statically significance.Serum uric acid was positively correlated with PASP and transverse diameter of right ventricular?r=0.386,r=0.876,P<0.05?,and serum uric acid was negatively correlated with PaO2?r=-0.375,P<0.05?.Uric acid/creatinine was positively correlated with PASP and transverse diameter of right ventricular?r=0.373,r=0.363,P<0.05?,and uric acid/creatinine was negatively correlated with PaO2?r=-0.576,P<0.05?.HIF-1?was positively correlated with serum uric acid,but not statistically significant?r=0.339,P=0.067?.HIF-1?was positively correlated with uric acid/creatinine,but not statistically significant?r=0.342,P=0.065?.Serum uric acid was positively correlated with PaCO2,but it was not statistically significant?r=0.087,P=0.648?.Uric acid/creatinine was positively correlated with PaCO2,but it was not statistically significant?r=0.187,P=0.323?.Conciusions:1.Compared with the group COPD,the patients with COPD-PH had shown the symptom of HIF-1?,blood uric acid and uric acid/creatinine elevation,which indicated that HIF-1?and blood uric acid are involved in the formation of pulmonary hypertension.2.In group COPD-PH:HIF-1?and serum uric acid levels are positively correlated with PASP and transverse diameter of right ventricular.Therefore,the determination of HIF-1?and serum uric acid can assess the severity of disease in patients with COPD complicated with pulmonary hypertension,which has important clinical significance for the evaluation and monitoring of patients'conditions.
