Objectives To study the evaluation value of platelet parameters in patients with ST elevation acute myocardial infarction.Methods A total of 113 patients with first-episode acute ST-segment elevation myocardial infarction who had been admitted to the emergency department without a history of anti-platelet medication were randomly selected to be included in the group.After admission,according to the intraoperative imaging results,the thrombotic load was determined to be divided into 79 patients in the light-thrombotic load group and 34 patients in the heavy-thrombotic load group,and the platelet parameters of the two groups were compared.After admission,According to the results of intraoperative angiography,the degree of thrombus load was determined and divided into 74 patients in the light thrombus load group and 18 patients in the heavy thrombus load group.Platelet parameters in the two groups were compared.At the same time,the ability of platelet parameters in STEMI patients to predict the degree of thrombus load in patients with acute myocardial infarction was evaluated by Logistic regression analysis and the curve of the subjects' working characteristics(ROC).Statistical analysis was performed with SPSS17.0 software package.Continuous variables are do normal inspection,Independent sample T test was used to compare the data sets.Mann-whitney U test was used for comparison between groups with non-normal distribution.The count data were expressed by composition ratio,chi-square test was used for comparison,and rank sum test was used for comparison.The predictive power of MPV?PDW?P-LCR?PCT and MPV/PLT for heavy thrombus load in STEMI patients was analyzed using ROC curves,P<0.05 was considered statistically significant.Results The mean platelet volume(MPV),platelet distribution width(PDW)and MPV/PLT ratio of patients with heavy thrombus load were higher than those with light thrombus load [MPV(12.18±1.51)vs(10.99±1.41),P<0.001;PDW(15.35±1.98)vs(13.33±2.25),MPV/PLT ratio(0.0536±0.0101)vs(0.0491±0.0148)].Multivariate Logistic regression analysis showed that the increased MPV,PDW and MPV/PLT ratio >0.0515 were independent risk factors for the occurrence of heavy thrombus load(MPV,PDW,MPV/PLT ratio were 1.689?1.369?1.187,and 95% confidence intervals were 64.10-81.10 ? 66.50-83.10 ? 48.30-67.20,P<0.05),and the difference was statistically significant.There was no significant difference in platelet ratio(p-lcr)or platelet pressure(PCT).Conclusions 1 Among patients with acute st-elevation myocardial infarction who had no history of antiplatelet medication,the ratio of platelet parameters MPV ? PDW and MPV/PLT in the heavy thromboembolic load group was higher than that in the light thromboembolic load group,and the difference was statistically significant.MPV?PDW and MPV/PLT were independent predictors of thrombotic load in STEMI patients.2 There was no statistically significant difference in platelet parameter PCT P-LCR between heavy and light thrombus load groups.Figure 1;Table 6;Reference 131.
