Aims:The aims of this study is to investigate the value of preoperative platelet-associated parameters(PLT,MPV,PDW and PLR)in the diagnosis of endometrial cancer and its correlation with clinicopathological features.Methods:In this study,the tested subjects were 240 patients with endometrial adenocarcinoma who underwent staging operation for endometrial cancer and postoperative paraffin pathology in the affiliated hospital of Yanbian university from January 2008 to October 2018,135 patients with endometrial polyp,and 113 patients with normal cases.According to nationality,postoperative pathological stage,pathological grade,presence or absence of deep muscle infiltration,vascular infiltration and cervical interstitial infiltration,these patients were divided into different groups,which were analyzed by age,BMI,PLT,PDW,MPV and PLR.Results:1.BMI,PLT and PLR of endometrial cancer patients in the same age group were significantly higher than those of endometrial polyp group and normal control group(P<0.05),while PDW of endometrial cancer patients was markedly lower than those of endometrial polyp group and normal control group(P<0.05).There was no significant difference in age or MPV among these three groups(P>0.05).2.The ROC curve analysis of PLT,PDW and PLR in the diagnosis of endometrial cancer showed that the optimal cut-off value of PLT was 269.5,with a sensitivity of 32.9%,a specificity of 82.3%,and an AUC of 0.583(95%CI 0.520-0.646)(P=0.012).The optimal cut-off value of PDW was 10.75,with a sensitivity of 27.9%,a specificity of 80.32%,and an AUC of 0.577(95%CI 0.541-0.640)(P=0.02).The optimal cut-off value of PLR was 129.34,with a sensitivity of 49.6%,a specificity of 75.20%,and an AUC of 0.641(95%Cl 0.582-0.701)(P<0.001).The sensitivity,specificity and AUC of combined diagnosis were 51.1%,72.6%,0.684(95%CI 0.618-0.750)(P<0.001).3.BMI of the Korean patients with endometrial cancer was remarkably lower than that of Han patients(P=0.003).There was no marked difference in age,PLT,MPV,PDW,PLR between Korean and Han patients(P>0.05).4.The preoperative PDW of Stage III patients was significantly lower than that of Stage ?-? patients(P=0.005),and the preoperative PLR was significantly higher than that of Stage?-? patients(P=0.004).There was no marked difference in preoperative PLT and MPV between Stagel-II patients and Stage ? patients(P>0.05).5.PLT and PLR were significantly increased in patients with vascular infiltration and cervical interstitial infiltration(P<0.001,P<0.001).P=0.002,P<0.001),PDW was significantly decreased(P=0.001,P<0.001),MPV difference was not statistically significant(P>0.05).PLT,MPV,PDW and PLR had no statistically significant difference in the pathological grading and the depth of muscle invasion in patients with endometrial cancer.Conclusion:1.Preoperative values of PLT,PDW and PLR have certain diagnostic value for endometrial cancer,and the combined diagnostic efficacy of the three is higher.Their were related to the vascular invasion and cervical interstitial invasion of endometrial cancer.2.The preoperative PDW and PLR values were correlated with the postoperative pathological staging of endometrial cancer.
