Aim: The purpose of this study is to explore the proper ice-salt bag compression time for analgesia and detumescence after knee arthroscopy.Range of motion and knee function score were used to determine the best ice-salt bag intervention time after knee arthroscopy. Methods: In this study,160 patients treated with knee arthroscopy from June 1,2015 to June 1,2017 were selected as subjects to support the analysis. The 160 patients were divided into four groups according to the using time of ice-salt bag after knee arthroscopy:12-hour group,24-hour group,36-hour group and 72-hour group. All groups were treated by using ice-salt bag discontinuously.Visual Analog Scale(VAS)was used to compare the analgesic effect of four groups after knee arthroscopy;Range of Motion(ROM)and Knee Society Score(KSS)were used to evaluate the recovery of knee joint after operation;hemoglobin content and hematocrit were utilized to evaluate the postoperative hemorrhage;and lower limb circumference measurements were used to compare knee joint swelling and complications after knee arthroscopy(subcutaneous ecchymosis, subcutaneous hematoma, deep vein thrombosis of lower limbs and skin infection of knee joint).Result:(1)The baseline data has no statistical difference in sex, age, height, weight and BMI between the four groups(P>0.05);(2)There was no significant difference in the left and right sides of the injured knee joint between the four groups(P>0.05);(3)No statistical difference in operation time, anesthesia time and incision length was found between the four groups(P>0.05)during the research.(4)There was no statistical difference in hemoglobin content in the groups before operation,1 day,3 days and 7 days after operation(P>0.05);(5)There was no statistical difference in hematocrit among the groups before operation,1 day,3 days and 7 days after operation(P>0.05);(6)There was no statistical difference in the range of motion(ROM)before operation,1 day,3 days and7 days after operation(P>0.05);(7)There was no statistical difference in the diameter of the affected limbs in the groups before operation,3 days and 7 days after operation(P>0.05),but there was significant difference in the perimeter of affected limbs in 1 day after operation from the other groups(P<0.05).The circumference of affected limbs in group A was larger than those in groups B,C and D(P<0.05);There was no significant difference between group B,group C and group D(P>0.05).(8)No statistical difference in VAS scores of affected limbs existed before and 7 days after surgery between the four groups(P>0.05).However,the VAS scores on 1 day after operation and 3 days after operation were significantly different(P<0.05).The VAS score of affected knee joint in group A was significantly higher than those in groups B,C and D(P<0.05),but there was significant difference in VAS score of affected knee joint between the four groups at 3days after operation(F=11.104,P<0.05).The VAS score of affected knee joint in 3 days after arthroscopy in group D was higher than those in groups A and B(P<0.05);similarly,the VAS score of affected knee joint in 3 days after operation in group C was higher than those in groups A and B(P<0.05).(9)There was no significant difference in KSS scores between the four groups before operation,1 day,3 days,7 days and 30 days after surgery(P>0.05).No significant difference was detected between group B,group C and group D(P>0.05).(10)For tension blister and subcutaneous ecchymosis, group A exhibits higher values than those of groups B,C and D(P<0.05).There was no statistical difference in incision infection, subcutaneous hematoma and DVT between these groups(P>0.05).Conclusion: Ice-salt bag intermittent treatment for 24 hours after knee arthroscopy is the best strategy. For such treatment less than 24 hours,no analgesic and detumescent effect exist.Incidence of postoperative pain will increase whenever ice-salt bag intermittent treatment is more than 24 hours.
