| Objective To explore the correlation between dry eye and rheumatoid arthritis activity.In order to have further understanding of the pathogenesis of dry eye with rheumatoid arthritis patients,and to provide help for diagnosis and treatment of dry eye patients associated rheumatoid arthritis in Inner Mongolia autonomous region.Methods The people who lived in Inner Mongolia region for a long time were recruited at Department of Rheumatism and Immunology and Myopia Laser Treatment Center of the Affiliated Hospital of Inner Mongolia Medical University between December 2016 and September 2018.They were divided into the simple dry eye group(160 cases,320 eyes)and dry eye with rheumatoid arthritis group(130 cases,260 eyes).The two groups respectively were performed Corvis ST to detect corneal biomechanics and Corvis ST inspection includes First Applanation Time(A1-time),Second Applanation Time(A2-time),First Applanation Length(A1-length),Second Applanation Length(A2-length),First Applanation Velocity(A1-V),Second Applanation Velocity(A2-V),Highest Concavity Time(HC-time),Maximum Deformation Amplitude(DA),Maximum Highest Concavity Radius(HC-radius),Intraocular Pressure(IOP),Corneal Center Thickness(CCT).The differences of the above parameters between the two groups were examined.All the subjects underwent breakup time(BUT),Schirmer ? test(S ? t),corneal staining score(CSS).At the same time,the rheumatoid arthritis activity related indicators of the dry eye patients such as erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factor(RF)and anti-cyclic peptide containing citrulline(anti-anti-CCP)were collected.DAS28 integral was used for the intensity grading of rheumatoid arthritis activity.The characteristics of corneal biomechanics,the indicators of dry eye and rheumatoid arthritis activity and the correlation between dry eye and rheumatoid arthritis activity were analyzed.Results The differences in age,sex,IOP and CCT were not significant between the two groups(all P>0.05).The dry eye with rheumatoid arthritisgroup had significantly lower A1-time and HC-time,but higher DA(P=0.01,0.01,and 0.03,respectively)compared with simple dry eye group.According to the clinical examination results,the CSS of the dry eye with rheumatoid arthritis group was higher(P<0.05),while BUT and Schirmer I test was significantly shorter(P<0.05)than that of the simple dry eye group.In addition,Schirmer I test was significantly different between patients with anti-anti-CCP positive and anti-anti-CCP negative in dry eye with rheumatoid arthritis group(P < 0.05).CSS in patients with positive RF factor was higher than that in patients with negative RF factor,and the difference was statistically significant(P < 0.05).BUT was shorter in patients with positive CRP and ESR than in patients with negative CRP and ESR(P<0.05).Pearson correlation coefficients demonstrated that DAS28 values were negatively correlated with Schirmer I test(r=-0.251,P=0.045).Conclusions Dry eye is associated with activity of rheumatoid arthritis in Inner Mongolia autonomous region.There are differences in corneal biomechanical properties between dry eye with rheumatoid arthritis group and simple dry eye group in Inner Mongolia autonomous region.The dry eye with rheumatoid arthritis group has a significantly shorter A1-time and HC-time,but larger DA compared with simple dry eye group.The degree of dry eye in patients with rheumatoid arthritis was significantly higher than that in patients with simple dry eye.The higher the activity of rheumatoid arthritis,the more severe the symptoms of dry eye.It is speculated that the activity of rheumatoid arthritis may affect the severity of dry eye.There is a correlation between the activity of rheumatoid arthritis and the severity of dry eye symptoms.Patients with rheumatoid arthritis should have a check-up of dry eye routinely,at the same time,patients with severe dry eye symptoms should be examined for rheumatoid arthritis.In this way,patients can do early intervention and early treatment. |
